Tilmicosin is a macrolide antibiotic primarily used in veterinary medicine for the treatment and prevention of respiratory diseases in livestock, particularly in cattle, sheep, and pigs. 

Mechanism of Action

Tilmicosin exerts its antibacterial effects by inhibiting bacterial protein synthesis. It binds to the 50S ribosomal subunit, blocking the translocation step of protein synthesis, which is essential for bacterial growth and reproduction. Additionally, tilmicosin has been noted to function as a calcium channel blocker, which can lead to cardiovascular effects, particularly in humans.

Treatment of bovine respiratory disease caused by Mannheimia (Pasteurella) haemolytica.
Prevention of enzootic pneumonia in sheep and other livestock.
Treatment of mycoplasmal infections in poultry, such as Mycoplasma gallisepticum.

Routes of Administration

Subcutaneous Injection: Most commonly used route for livestock.
Intramuscular Injection: Less frequently used but still applicable.

Common Side Effects: Injection site pain, swelling, and hypersensitivity reactions.
Serious Adverse Effects: Cardiotoxicity in humans can occur with accidental exposure or overdose, leading to tachycardia, bradycardia, hypotension, and even death. Reports indicate that doses as low as 1 mL can be fatal in humans.
Toxicity Symptoms: Increased heart rate, pulmonary edema, and depression have been noted following tilmicosin administration in animals.

Metabolism: Primarily metabolized in the liver with the formation of metabolites such as T-1 (N-desmethyl tilmicosin) and T-2.
Excretion: Mainly excreted via feces; minimal renal clearance has been observed.
