Apixaban, sold under the brand name Eliquis, is an anticoagulant medication and a direct factor Xa inhibitor. It's used to treat and prevent blood clots and to prevent stroke in people with nonvalvular atrial fibrillation. Apixaban is an alternative to warfarin with fewer medication interactions, and it doesn't require regular monitoring through blood tests or dietary restrictions.

Mechanism of Action

Apixaban is a potent, direct, oral, reversible, and highly selective inhibitor of Factor Xa (FXa). By inhibiting both free and clot-bound FXa, as well as prothrombinase activity, apixaban prevents thrombus formation and inhibits clot growth. It decreases thrombin generation and, indirectly, inhibits platelet aggregation induced by thrombin. Apixaban does not require antithrombin III for antithrombotic activity.

Applications

Apixaban is approved for clinical use in several thromboembolic disorders, including:
Reduction of stroke risk in non-valvular atrial fibrillation (NVAF).
Thromboprophylaxis following hip or knee replacement surgery.
Treatment of deep vein thrombosis (DVT) or pulmonary embolism (PE).
Prevention of recurrent DVT and PE.

Safety and Side Effects

While apixaban has enormous benefits, it can cause you to bleed more than normal.
Common side effects:
Bleeding, including nosebleeds, easy bruising, heavy menstrual bleeding, and bleeding of the gums when brushing teeth.
Serious side effects:
Serious bleeding: This can be dangerous and needs urgent medical attention. Contact 111 for advice if you have red urine or black stool, bruises that happen for no reason or are larger than expected, or bruises that keep growing in size. Go to A&E immediately if you experience nosebleeds that last longer than 10 minutes despite attempts to stop them, blood in your vomit, coughing up blood, severe headaches, or bleeding from a cut or injury that won't stop. If you experience serious bleeding, stop taking apixaban.
Bleeding in the brain: Very rarely, apixaban can lead to bleeding in the brain. Call 999 or go to A&E if you experience a sudden severe headache, fits or seizures, sudden loss of vision or blurred vision, or a feeling of numbness or tingling in your arms or legs.
Allergic reactions: Stop taking apixaban and get help right away if you have symptoms of a serious allergic reaction, such as breathing problems or wheezing, racing heart, fever or general ill feeling, swollen lymph nodes, swelling of the face, lips, mouth, tongue, or throat, trouble swallowing or throat tightness, itching, skin rash, or pale red bumps on the skin (hives), nausea or vomiting, dizziness, feeling lightheaded, or fainting, stomach cramps, or joint pain.
Apixaban may increase the risk of blood clot around the spinal cord if you undergo a spinal tap or receive spinal anesthesia (epidural), which could cause long-term paralysis.
Apixaban may cause you to bleed more easily, especially if you have a bleeding disorder. Taking apixaban may increase the risk of bleeding while you are pregnant or during deliver.

Pharmacokinetics

Absorption: Apixaban is rapidly absorbed, with maximum concentration occurring 3–4 hours after oral administration. The absolute oral bioavailability of apixaban is ~50%. Food does not have a clinically meaningful impact on bioavailability.
Distribution: The volume of distribution is approximately 21L. Protein binding is 92-94%.
Metabolism: Apixaban is mainly metabolized by CYP3A4 and to a lesser extent by CYP1A2, CYP2C8, CYP2C9, CYP2C19, and CYP2J2. 50% of the orally administered dose is excreted as the unchanged parent compound, however 25% of the dose is excreted as O-demethyl apixaban sulfate. All apixaban metabolites account for approximately 32% of the excreted dose.
Elimination: Elimination occurs via multiple pathways including metabolism, biliary excretion, and direct intestinal excretion, with approximately 27% of total apixaban clearance occurring via renal excretion. Apixaban has a half-life of approximately 12 hours.
