Description	Enzalutamide, also known as MDV3100, is an orally bioavailable, organic, non-steroidal small molecule targeting the androgen receptor (AR) with potential antineoplastic activity. Through a mechanism that is reported to be different from other approved AR antagonists, selective androgen receptor modulator MDV3100 inhibits the activity of prostate cancer cell ARs, which may result in a reduction in prostate cancer cell proliferation and, correspondingly, a reduction in the serum prostate specific antigen (PSA) level. In August 2012, the United States (U.S.) Food and Drug Administration (FDA) approved enzalutamide for the treatment of castration-resistant prostate cancer.

Mechanism of Action

Enzalutamide is an androgen receptor inhibitor. It works by competitively binding to the androgen receptor, preventing androgen hormones such as testosterone from binding to the receptor. This blocks the activation of the androgen receptor and subsequent downstream signaling pathways that promote the growth and survival of prostate cancer cells. In this way, it can effectively inhibit the proliferation and spread of cancer cells.

Clinical Applications

Metastatic Castration-Resistant Prostate Cancer (mCRPC): Enzalutamide is widely used in the treatment of mCRPC. It has been shown to significantly prolong survival, delay the progression of the disease, and improve the quality of life of patients. For example, in some clinical trials, patients treated with enzalutamide had a longer time to disease progression and a lower risk of death compared to those in the control group.

Non-Metastatic Castration-Resistant Prostate Cancer (nmCRPC): It is also indicated for the treatment of nmCRPC. In patients with high-risk nmCRPC, enzalutamide can delay the occurrence of metastasis and prolong the time to metastasis-free survival.

Dosage and Administration

The recommended dose of enzalutamide is 160 mg (four 40 mg tablets) taken orally once daily. It can be taken with or without food. It is important to take the tablets whole and not crush, dissolve, or chew them. Patients should continue to take enzalutamide until disease progression or unacceptable toxicity occurs.

Side Effects

Common Side Effects: These may include fatigue, weakness, hot flashes, nausea, diarrhea, constipation, and decreased appetite. Some patients may also experience back pain, joint pain, or muscle pain.

Serious Side Effects: In some cases, enzalutamide can cause more serious side effects such as seizures, high blood pressure, and liver toxicity. It may also lead to increased risk of fractures due to its effect on bone metabolism. Regular monitoring of blood pressure, liver function, and bone health is usually required during treatment.
