Pharmacological action

Rosuvastatin is a selective HMG-CoA reductase inhibitor. HMG-CoA reductase inhibitors are rate-limiting enzymes that convert 3-hydroxy-3-methylglutaryl-CoA to valerate, A precursor of cholesterol. The main site of action of rosuvastatin is the liver - a targeted organ for lowering cholesterol. Rosuvastatin increased the number of receptors on the surface of liver LDL cells, promoted the absorption and catabolism of LDL, inhibited the liver synthesis of VLDL, and thus reduced the total number of VLDL and LDL particles. In homozygous and heterozygous patients with familial hypercholesterolemia, non-familial hypercholesterolemia, and mixed dyslipidemia, rosuvastatin can reduce the levels of total cholesterol, LDL-C, ApChemicalbookoB, and non-HDL-C. Rosuvastatin also decreased TG and increased HDL-C levels. In patients with simple hypertriglyceridemia, rosuvastatin can reduce total cholesterol, LDL-C, VLDL-C, ApoB, non-HDL-C, TG levels, and increase HDL-C levels. Preclinical data based on general safety pharmacology, repeated dose toxicity, potential genotoxicity, and carcinogenicity studies do not show specific toxicity to humans. In studies of rats before and after birth, the reproductive toxicity of rosuvastatin was evident, reducing litter size, weight, and birth rate. These reproductive toxicity phenomena were observed when female rats were given toxic doses that made their systemic exposure several times higher than the therapeutic exposure level.

Pharmacokinetics

This product is orally absorbed by the liver in large quantities, with a volume of distribution of about 134L, and the blood concentration reaches its peak after 3 to 5 hours. The absolute bioavailability is 20%. The plasma protein binding rate (mainly albumin) is about 90%. About 90% of the dose of rosuvastatin is excreted in its original form in the stool (both absorbed and unabsorbed active substances), and the remainder is excreted in the urine. Chemicalbook is present in about 5% of urine. The plasma clearance half-life is about 19 hours. The clearance half-life did not increase with dose increase. The geometric mean plasma clearance was approximately 50L/ h (coefficient of variation 21.7%). As with other HMG-CoA reductase inhibitors, hepatic uptake of rosuvastatin involves the membrane transporter OATP-C. This transporter is important in hepatic clearance of rosuvastatin.
