Itraconazole is a broad-spectrum triazole antifungal agent used to treat a variety of fungal infections. Its primary mechanism of action involves inhibition of the fungal cytochrome P450 enzyme lanosterol 14-α-demethylase, which blocks the conversion of lanosterol to ergosterol, an essential component of the fungal cell membrane. This disruption leads to increased membrane permeability, altered membrane-bound enzyme activity, and impaired fungal cell growth and viability.

Mechanism of Action

Inhibits lanosterol 14-α-demethylase, preventing ergosterol synthesis.
Causes accumulation of toxic sterol intermediates, disrupting fungal cell membrane integrity.
Also inhibits fungal cytochrome c oxidative and peroxidative enzymes, further impairing membrane function.
Unique among azoles, itraconazole also inhibits the hedgehog signaling pathway and angiogenesis, though these effects are unrelated to its antifungal action.

Applications

Itraconazole is indicated for treatment of:
Blastomycosis (pulmonary and extrapulmonary)
Histoplasmosis, including chronic cavitary and disseminated forms
Aspergillosis in patients intolerant or refractory to amphotericin B
Onychomycosis of toenails and fingernails caused by dermatophytes
Oropharyngeal and esophageal candidiasis (oral solution)
It is used in both immunocompromised and non-immunocompromised patients.

Routes of Administration

Oral capsules and tablets
Oral solution (especially for candidiasis)
Intravenous formulation was previously available but discontinued in the US.

Pharmacokinetics

Oral bioavailability varies and is improved when taken with fatty meals.
Metabolized extensively by hepatic CYP3A4 enzymes; itraconazole is also a CYP3A4 substrate and inhibitor.
Half-life ranges approximately 34 hours, supporting once- or twice-daily dosing.
Serum levels can vary widely among patients; newer formulations have improved bioavailability and reduced food effect.
Itraconazole inhibits P-glycoprotein, affecting the pharmacokinetics of other drugs.
