    Introduction

    Use is generally limited to short-term treatment (typically ≤15 days) due to its safety profile.

    Dosage and Administration

    The typical adult oral dose is 100 mg taken twice daily, after meals to reduce gastrointestinal side effects. It is available in various forms including tablets, dispersible granules, and suspensions for pediatric use (only in some countries).

    Topical formulations (gels or creams) may be applied to the affected area 2–3 times daily, depending on the product. Rectal suppositories (in certain markets) are also occasionally used.

    Nimesulide is not recommended for use in children under 12 years of age or in elderly patients with liver or kidney problems.

    Pharmacokinetics

    After oral administration, nimesulide is rapidly absorbed, with peak plasma concentrations reached within 1.5 to 2.5 hours. It is extensively bound to plasma proteins (around 99%) and primarily metabolized in the liver to its main active metabolite, 4-hydroxy nimesulide.

    Its elimination half-life is approximately 1.8–4.7 hours. The drug and its metabolites are mainly excreted through urine (about 50%) and feces.

    Common Side Effects

    Commonly observed adverse effects include:

    Gastrointestinal discomfort such as nausea, diarrhea, dyspepsia, and abdominal pain

    Headache

    Drowsiness or dizziness

    Skin rashes or itching

    Elevated liver enzymes (indicative of liver stress)

    To reduce the risk of stomach upset, nimesulide is usually taken after food.

    Serious or Rare Adverse Reactions

    Serious adverse reactions, although rare, include:

    Hepatotoxicity (liver damage), including hepatitis, jaundice, and liver failure

    Gastrointestinal bleeding or ulcer (less frequently than with non-selective NSAIDs)
