The pharmacological action of lactulose is unique and occurs almost entirely within the colon. As a non-absorbable compound, it passes through the small intestine unchanged. Upon reaching the large intestine, it serves as a substrate for saccharolytic fermentation by the resident gut flora. This bacterial metabolism breaks lactulose down into low molecular weight organic acids, primarily lactic acid and acetic acid, along with small amounts of other short-chain fatty acids and gases.

The primary clinical applications of lactulose are focused on the treatment and management of two distinct conditions. Its most critical use is in the treatment and prevention of hepatic encephalopathy, a neuropsychiatric complication of liver failure. The acids produced from its fermentation lower the colonic pH, which traps ammonia in the form of ammonium ions that are then excreted in the stool, thereby reducing blood ammonia levels. Secondly, it is widely used as an osmotic laxative for the treatment of chronic constipation, as the acidic metabolites draw water into the colon by osmosis and stimulate peristalsis.

Lactulose is administered almost exclusively via the oral route, available as a syrup, solution, or crystalline powder for reconstitution. The dosage is highly indication-dependent; a smaller dose is typically used for constipation, while a larger, more frequent dose is required for the management of hepatic encephalopathy. Its effects are not immediate, with a laxative effect typically occurring within 24 to 48 hours, as it relies on the process of bacterial fermentation.

The safety profile of lactulose is generally favorable, contributing to its widespread and long-term use. The most common adverse effects are related to its mechanism of action in the gut and include flatulence, belching, abdominal cramping, and discomfort, particularly at the initiation of therapy. These symptoms often diminish with continued use. Excessive dosage can lead to severe diarrhea and resulting electrolyte imbalances. Its safety in pregnancy and for pediatric use is well-regarded, though it should always be used under medical guidance.
