    Octenidine dihydrochloride is a cationic bispyridine antiseptic agent widely used in clinical practice for the prevention and treatment of local infections . Chemically designated as N-octyl-1-[10-(4-octyliminopyridin-1-yl)decyl]pyridin-4-imine dihydrochloride, it has a molecular formula of C36H64Cl2N4 and a molecular weight of approximately 623.8 g/mol . Developed in the 1980s, this compound was specifically designed as an alternative to chlorhexidine, offering a broader antimicrobial spectrum with comparable or superior tissue tolerance .

     

    The antimicrobial mechanism of octenidine is fundamentally based on its physicochemical interaction with microbial cell membranes. As a cationic surfactant, its positively charged molecules are electrostatically attracted to the negatively charged components of bacterial and fungal cell envelopes . This interaction disrupts membrane integrity, leading to leakage of intracellular components and eventual cell death . Unlike many antibiotics that target specific metabolic pathways, this membrane-active mechanism makes the development of resistance highly unlikely, as it would require complete restructuring of the microbial cell envelope . Recent research has further elucidated that octenidine can impair ergosterol biosynthesis in fungi and induce reactive oxygen species generation, compromising cell membrane integrity in pathogens such as Candida albicans .

     

    Octenidine demonstrates a remarkably broad spectrum of antimicrobial activity with rapid onset. It is effective against Gram-positive and Gram-negative bacteria, including multidrug-resistant strains such as methicillin-resistant Staphylococcus aureus (MRSA), as well as fungi and some enveloped viruses . For most susceptible species, minimum inhibitory concentrations range from approximately 1 to several micrograms per milliliter . One of its most valuable clinical properties is its potent antibiofilm activity; it not only inhibits the formation of biofilms but also disrupts mature biofilms, which are notoriously difficult to eradicate with conventional antimicrobials . The required contact time for microbicidal action is rapid, often achieving significant reduction within one minute .

     

    Clinically, octenidine is employed in a wide variety of applications due to its favorable efficacy and safety profile. It is considered a first-choice antiseptic for critically colonized wounds, infection-prone wounds, burns, and wounds colonized by multidrug-resistant pathogens, as well as for the prevention of surgical site infections . Additional applications include umbilical stump care, treatment of oral infections, skin and mucosal candidiasis, and bacterial vaginosis . Formulations typically contain octenidine in aqueous solution, often combined with phenoxyethanol as a solvent and preservative, though research into lipid-based formulations aims to reduce the irritant potential of phenoxyethanol for sensitive applications .

     

    The safety profile of octenidine is well-established, with a high biocompatibility index indicating a wide margin between antimicrobial efficacy and cytotoxic effects on human cells . Its Clinical Efficiency of MIC (CEMIC) index is classified as excellent for most pathogens, meaning that clinically used concentrations are substantially higher than the minimum inhibitory concentrations required . The risk of bacterial adaptation or resistance development is considered very low for most species, with the Karpinski Adaptation Index remaining below 0.2 for the vast majority of organisms . However, as with any potent antiseptic, it should be used according to established guidelines to maintain its efficacy and safety profile in clinical practice .
