Phenylbutazone More Info

Phenylbutazone is a pyrazolone derivative and belongs to the class of nonsteroidal anti-inflammatory drugs (NSAIDs). Due to its potent anti-inflammatory effects and significant adverse effects, it is now rarely used in human medicine; however, it is still employed in veterinary medicine (particularly for horses) to treat musculoskeletal disorders.

Mechanism of Action:

Phentazone exerts its anti-inflammatory, analgesic, and antipyretic effects through multiple mechanisms:

Inhibition of cyclooxygenase (COX): This is its primary mechanism of action. It irreversibly binds to and inactivates prostaglandin H synthases (COX-1 and COX-2), thereby blocking the conversion of arachidonic acid into prostaglandins. Prostaglandins are key mediators of inflammation, pain, and fever; reducing their production effectively alleviates these symptoms.

Inhibition of prostacyclin synthase: Phenylbutazone also inhibits prostacyclin synthase, further influencing inflammatory pathways.

Promotion of uric acid excretion: When used in high doses, phenylbutazone reduces the reabsorption of uric acid by the renal tubules, promoting its excretion in the urine, which is particularly valuable for the treatment of acute gout.

Applications ：

The primary use of phenylbutazone has shifted significantly and is now concentrated in the veterinary field.

Musculoskeletal disorders: Used to relieve pain and inflammation associated with osteoarthritis, bursitis, tendinitis, and laminitis in horses.

Soft tissue inflammation: Used to treat myositis, postoperative inflammation, and other soft tissue injuries.

Symptomatic treatment: Used as an antipyretic for conditions such as viral respiratory infections.

Metabolic management: Recent studies indicate that, in addition to pain relief, phenylbutazone can lower insulin levels in horses with insulin resistance by modulating tissue insulin sensitivity, suggesting broader potential for managing related metabolic disorders.

Safety and Side Effects

The side effects of phenylbutazone incidence rate is about 10%~20%, gastrointestinal irritation can cause nausea, vomiting, abdominal pain, constipation, overdose can cause peptic ulcer, blood in the stool. Damages also occur in other systems, such as rash, dizziness, hematuria, hepatitis, etc. Inhibit the bone marrow caused by neutropenia, or even aplastic anemia, such as the timely withdrawal is more recoverable and therefore should be ground check blood, no effect by 1 week, should not be reused. Patients with weak liver, osteoporosis, and kidney as well as drug allergy history are contraindicated or appropriate caution.
