It is a biocompatible excipient with a slightly sweet taste, primarily used in the pharmaceutical industry to adjust pH and enhance the solubility of poorly water-soluble drugs, particularly iodinated contrast media for X-rays and MRI

Mechanism of Action

As an excipient, meglumine itself is generally considered biologically inert, acting as a counter-ion to form stable salts with acidic drugs (e.g., diatrizoate, gadoterate) to increase their aqueous solubility . However, recent studies indicate it has intrinsic bioactivity: it acts as a weak organic base and has been shown to stimulate the AMPK-related enzyme SNARK (NUAK2), which plays a role in glucose uptake and muscle contraction .

Applications

Diagnostic Imaging (Primary use): Essential component in contrast agents for X-ray/CT (e.g., diatrizoate meglumine) and MRI (e.g., gadoterate meglumine) due to its high water solubility and low toxicity .

Therapeutic: Used as meglumine antimoniate, the first-line treatment for leishmaniasis (a parasitic disease) .

Metabolic Research: Emerging evidence suggests potential applications in improving glycemic control and muscle stamina in metabolic syndrome models .

Safety and Side Effects

General Safety: It is generally regarded as safe when used as an excipient. In animals, it lacks the gastrointestinal side effects (diarrhea) common to sorbitol .

Imaging Risks: Side effects are usually linked to the active drug or contrast agent, not meglumine itself. However, in contrast media, risks include Nephrogenic Systemic Fibrosis (NSF) in patients with kidney failure (for gadolinium agents) and hypersensitivity reactions .
