Description	Panobinostat is a potent inhibitor of all histone deacetylases (HDACs), with Ki values ranging from 0.6 to 31 nM for HDAC1-11. Panobinostat is a potent pan–histone deacetylase (HDAC) inhibitor with low-nanomolar activity against class I, II, and IV HDACs (IC₅₀ values typically 5–50 nM). In cancer cell lines, it induces histone hyperacetylation, cell cycle arrest, apoptosis, and inhibition of proliferation at low nanomolar concentrations across hematologic and solid tumor models. It also promotes accumulation of acetylated non-histone proteins such as Hsp90 and p53. In vivo, panobinostat demonstrates strong antitumor activity in xenograft models of multiple myeloma, lymphoma, and solid tumors, producing dose-dependent tumor growth inhibition and enhanced survival. Combination studies show synergistic effects with proteasome inhibitors and chemotherapeutic agents, particularly in myeloma models.

Mechanism of Action

Panobinostat works by inhibiting the enzymatic activity of histone deacetylases (HDACs) . This inhibition leads to the accumulation of acetylated histones and other proteins, which in turn alters chromatin structure and gene expression . The resulting changes induce cell cycle arrest and apoptosis (programmed cell death) in cancer cells.

Applications

Primary Indication: In combination with bortezomib and dexamethasone for the treatment of patients with relapsed or refractory multiple myeloma who have received at least two prior standard therapies .

Other Research: Being investigated for other hematological malignancies, solid tumors, and non-oncological conditions such as HIV latency reversal.

Safety and Side Effects

Panobinostat carries a Boxed Warning for severe diarrhea and fatal cardiac events .

Very Common Side Effects: Diarrhea (severe in 25% of patients), fatigue, nausea, peripheral edema, decreased appetite, fever, and vomiting .

Serious Risks:

Myelosuppression: Severe thrombocytopenia (67%), neutropenia (34%), and anemia are common, requiring frequent blood count monitoring .

Infections: Severe infections, including pneumonia and invasive fungal infections, have been reported .

Hepatotoxicity: Elevations in liver enzymes and bilirubin can occur; liver function must be monitored regularly .

Cardiac Toxicities: Can cause severe arrhythmias and ECG changes; electrolytes and ECG must be checked before and during treatment .

Embryo-Fetal Toxicity: Can cause fetal harm; effective contraception is required for both males and females during and after treatment.
