    Description

Erdafitinib, also known as JNJ-42756493, is a potent and selective orally bioavailable, pan fibroblast growth factor receptor (FGFR) inhibitor with potential antineoplastic activity. Upon oral administration, JNJ-42756493 binds to and inhibits FGFR, which may result in the inhibition of FGFR-related signal transduction pathways and thus the inhibition of tumor cell proliferation and tumor cell death in FGFR-overexpressing tumor cells. FGFR, upregulated in many tumor cell types, is a receptor tyrosine kinase essential to tumor cell proliferation, differentiation and survival.

    Panthenyl Triacetate More Info

Erdafitinib (brand name Balversa) is a kinase inhibitor that represents a new molecular entity . It is manufactured by Janssen (Johnson & Johnson's pharmaceutical division) .

    Mechanism of Action

Erdafitinib is a small-molecule, pan-fibroblast growth factor receptor (FGFR) tyrosine kinase inhibitor that inhibits all members of the FGFR family . Specifically, it binds to and inhibits the enzymatic activity of FGFR1, FGFR2, FGFR3, and FGFR4 . Genetic changes to the FGFR3 gene can produce an abnormal FGFR3 protein that drives uncontrolled growth of cancer cells; erdafitinib blocks the activity of this abnormal FGFR3 on the surface of cancer cells, stopping its function and thereby slowing the growth and spread of the cancer .

    Approved Indications

Erdafitinib is indicated for the treatment of adults with locally advanced or metastatic urothelial carcinoma (bladder cancer) that harbors susceptible FGFR3 or FGFR2 genetic alterations, particularly in patients who have progressed during or following at least one line of prior platinum-containing chemotherapy . It was the first FGFR kinase inhibitor approved by the U.S. FDA, receiving accelerated approval in April 2019 . Its approval also marked the first approval of a drug specifically targeting genetic alterations in the FGFR pathway .

    Efficacy

In clinical trials, erdafitinib demonstrated efficacy in patients with locally advanced or metastatic urothelial carcinoma, with an objective response rate (ORR) of 32.2% and a median duration of response of 5.4 months . In a phase III randomized controlled open-label study, Balversa showed superiority over chemotherapy in overall survival and progression-free survival .

    Dosage Forms

Balversa is available as 3 mg, 4 mg, and 5 mg film-coated tablets for oral use . The recommended starting dose is 8 mg taken once daily .

    Common Side Effects

The most common adverse reactions include hyperphosphataemia (elevated phosphate levels), diarrhoea, and stomatitis . Ocular issues such as dry eye, corneal and retinal problems may also occur .

    Regulatory Status

In June 2024, the EMA's CHMP adopted a positive opinion recommending marketing authorisation for Balversa in the treatment of urothelial carcinoma harboring susceptible FGFR3 genetic alterations.

    ECHEMI Editorial Reference
Erdafitinib (JNJ-42756493) is a potent and orally available FGFR family inhibitor; inhibits FGFR1/2/3/4 with IC50s of 1.2, 2.5, 3.0 and 5.7 nM, respectively.
In early April of 2019, the US FDA approved Janssen Pharmaceutical Companies' brand name Balversa (erdafitinib) as the first-ever fibroblast growth factor receptor (FGFR) kinase inhibitor indicated for patients with locally advanced or metastatic urothelial carcinoma, with susceptible FGFR3 or FGFR2 genetic alterations, that has progressed during or following platinum-containing chemotherapy, including within 12 months of neoadjuvant or adjuvant platinum-containing chemotherapy. At the same time, the FDA also approved the therascreen FGFR RGQ RT-PCR Kit (Qiagen) for utilization as a companion diagnostic with erdafitinib for selecting patients for the indicated therapy. Erdafitinib's innovation lies in the fact that it is the first personalized treatment targeting susceptible FGFR genetic alterations for patients with metastatic bladder cancer, which demonstrates the design of erdafitinib in developing more personalized and precision medicines with the capacity to target cancer treatment to a patient's specific genetic mutation. Considering urothelial cancer is statistically the fourth most common kind of cancer in the world, the introduction of erdafitinib offers a welcome new option in the ever-expanding therapeutic tool kit to treat such prevalent medical conditions. Nevertheless, although erdafitinib was granted Breakthrough Therapy designation and Accelerated Approval from the FDA so as to allow the agency to focus on and expedite the approval process for a medication indicated for a serious condition that fills an unmet medical need using clinical trial data that is believed to predict a genuine clinical benefit for patients with the given condition, such designations mean further ongoing clinical trials are necessary to confirm the clinical benefit of erdafitinib going forward.|Erdafitinib is an oral small molecule inhibitor of fibroblast growth factor receptors (FGFR) 1 to 4 that is used in the therapy of locally advanced, unresectable or metastatic urothelial carcinoma. Erdafitinib has been associated with a high rate of serum enzyme elevations during therapy, but has not been linked to cases of clinically apparent acute liver injury.|Erdafitinib is an orally bioavailable, pan fibroblast growth factor receptor (FGFR) inhibitor with potential antineoplastic activity. Upon oral administration, erdafitinib binds to and inhibits FGFR, which may result in the inhibition of FGFR-related signal transduction pathways and thus the inhibition of tumor cell proliferation and tumor cell death in FGFR-overexpressing tumor cells. Status:

Approved (FDA accelerated approval 2019-04-12; converted to regular approval 2024-01-19; EMA approval 2024-08)

    Mechanism:

Erdafitinib is an orally bioavailable, functionally selective small-molecule inhibitor of fibroblast growth factor receptors 1-4 (FGFR1-4). It inhibits FGFR tyrosine kinase activity (IC50 1.2-5.7 nM), blocking downstream RAS-MAPK/ERK and PI3K-AKT signaling that drives proliferation and survival of tumor cells harboring activating FGFR alterations (mutations, amplifications, or fusions). Rapid lysosomal uptake contributes to prolonged inhibition of FGFR signaling.
