No formal studies of interactions between bevacizumab and antineoplastic agents have been conducted. In Study 1, patients were given irinotecan/5-FU/CF (IV IFL) with or without bevacizumab. Irinotecan concentrations were the same in patients receiving IV IFL alone and in combination with bevacizumab. However, concentrations of irinotecan's active metabolite SN38 were, on average, 33% higher in patients receiving IFL plus bevacizumab than in patients receiving IV IFL alone. In Study 1, the incidence of grade 3-4 diarrhea and neutropenia was higher in patients receiving IV IFL plus bevacizumab, but the extent of the effect of increased SN38 levels caused by the combination of bevacizumab and irinotecan is unclear due to the heterogeneity of the patients enrolled and the limited number of samples. Carcinogenicity, Mutagenicity, and Impairment of Fertility. No data are available on the carcinogenicity of bevacizumab in humans or animals. Bevacizumab may impair fertility. After continuous administration of bevacizumab at a dose of 10 or 50 mg/kg for 13 or 26 weeks, dose-related changes in ovarian and uterine weights, endometrial proliferation, reduced number of menstrual cycles, and follicular block and corpus luteum loss were found in female macaques. After discontinuation of the drug and a recovery period of 4 to 12 weeks, the high-dose group was examined, and the results of the examination of two female macaques in the planned recovery group suggested that the damage was reversible. After a 12-week recovery period, the follicular block disappeared, but there was still a moderate reduction in ovarian weight, the reduction in endometrial proliferation disappeared, but the reduction in uterine weight was still significant, and one of the two macaques still had corpus luteum loss and a reduced number of menstrual cycles (67%).

In two clinical studies of metastatic renal cell carcinoma, microangiolytic hemolytic anemia (MAHA) was reported in 7 of 19 patients treated with bevacizumab (10 mg/kg every 2 weeks) in combination with sunitinib malate (50 mg daily); the safety and efficacy of combined therapy with radiation therapy and bevacizumab have not yet been established.
