Aficamten Beats Metoprolol in Obstructive HCM, Strengthening Case Ahead of FDA Decision

Cytokinetics’ aficamten improved symptoms and cardiopulmonary performance more than metoprolol in a 175-patient head-to-head study of obstructive hypertrophic cardiomyopathy (oHCM), with detailed data unveiled at the ESC Congress (Madrid).

 

After 24 weeks, patients on aficamten showed an average +1.1 mL/kg/min increase in peak VO₂, while the metoprolol arm declined. >50% of aficamten patients reported better physical limitation scores vs 26% on metoprolol, alongside improved quality of life. Analysts suggest aficamten could displace beta-blockers as first-line in oHCM. The U.S. FDA is expected to decide by December 2025.
