Function and Efficacy

Berberine hydrochloride and trimethoprim have synergistic antibacterial effects. Berberine hydrochloride has inhibitory effects on a variety of Gram-positive and Gram-negative bacteria in vitro, among which hemolytic streptococci, Staphylococcus aureus, Vibrio cholerae, Neisseria meningitidis, Shigella, Salmonella typhi, Corynebacterium diphtheriae, etc. have stronger inhibitory effects. It also has a certain inhibitory effect on amoeba. Trimethoprim mainly interferes with bacterial folate metabolism, selectively inhibits dihydrofolate reductase, and prevents nucleic acid synthesis. Therefore, the antibacterial effect of the two combined is stronger than that of a single drug, and the number of drug-resistant strains is reduced.

Newborns and infants under 2 years old should not use this drug; children are in the growth and development period, and their liver and kidney functions are not yet perfect, so the dosage should be reduced as appropriate. Precautions for pregnancy and lactation: 1. Since trimethoprim can cross the placental barrier, the blood concentration in the fetal circulation and amniotic fluid is close to the maternal blood concentration. Therefore, the pros and cons of this product should be weighed during pregnancy to decide whether to use it. 2. Since trimethoprim can be secreted into breast milk, its blood concentration is high, which may interfere with the folic acid metabolism of breastfeeding infants, although the problem has not been confirmed in humans. However, the use of this product for breastfeeding mothers must be decided after weighing the pros and cons. Precautions for the elderly: Elderly patients have reduced resistance and weakened metabolic capacity. When taking this product, they should pay attention to blood tests. Drug Interactions

1. The interaction between berberine hydrochloride and other drugs is still unclear. 2. Trimethoprim: (1) When used simultaneously with bone marrow suppressants, the chance of leukopenia and thrombocytopenia increases. (2) When used simultaneously with dapsone, the blood concentrations of both drugs increase. The increase in blood concentration of dapsone increases and aggravates adverse reactions, especially the occurrence of methemoglobinemia. (3) When anti-tumor drugs and 2,4-diaminopyrimidine drugs are used simultaneously, it is also not advisable to use this product between courses of treatment with other folic acid antagonists, because it may cause bone marrow aplasia or megaloblastic anemia. (4) When used simultaneously with reserpine, trimethoprim elimination may be increased and plasma half-life shortened. (5) When used simultaneously with cyclosporine, nephrotoxicity may be increased. (6) It interferes with the hepatic metabolism of phenytoin, increases the half-life of phenytoin, and reduces its clearance rate. (7) When used simultaneously with procainamide, renal clearance may be reduced and the blood concentration of procainamide metabolites may be increased. (8) It inhibits the metabolism of warfarin. And enhance its anticoagulant effect.
