Function and Efficacy

Somatostatin is a synthetic cyclic fourteen amino acid peptide, which is completely identical to natural somatostatin in chemical structure and action. Intravenous injection of silatin can inhibit the secretion of growth hormone, thyroid stimulating hormone, insulin and glucagon, and inhibit the secretion of gastric acid. It also affects the absorption, motility, visceral blood flow and nutritional function of the gastrointestinal tract. Physiologically, somatostatin is mainly present in the hypothalamus and gastrointestinal tract. Somatostatin can inhibit the secretion of gastrin, gastric acid and pepsin, thereby treating gastrointestinal bleeding. Moreover, somatostatin can significantly reduce the blood flow of visceral organs without causing significant changes in systemic arterial blood pressure, so it has clinical value in the treatment of esophageal varicose bleeding. Somatostatin can reduce the endocrine and exocrine secretion of the pancreas, thereby effectively preventing and treating complications after pancreatic surgery. Somatostatin can also inhibit the secretion of glucagon and can effectively treat diabetic ketoacidosis.

Appearance

This product is white freeze-dried powder.

Indication

Severe acute esophageal varicose bleeding. Severe acute gastric or duodenal ulcer bleeding, or complicated by acute erosive gastritis or hemorrhagic gastritis. Adjuvant treatment of pancreatic, biliary and intestinal fistulas. Prevention and treatment of complications after pancreatic surgery. Adjuvant treatment of diabetic ketoacidosis.

Usage and Dosage

The lyophilized powder must be dissolved in saline before use. Stamista is administered intravenously, either by slow bolus injection (3 to 5 minutes) of 250 micrograms or by continuous drip at a rate of 250 micrograms per hour (equivalent to approximately 3.5 micrograms per kilogram of body weight per hour). For continuous drip administration, a 3 mg vial of Stamista is used to prepare a 12-hour supply of solution, the solvent of which can be either saline or 5% glucose solution. The infusion volume should be adjusted to 250 micrograms per hour, and an infusion syringe is recommended. For the treatment of severe acute upper gastrointestinal bleeding (including esophageal variceal bleeding): It is recommended to first slowly inject 250 micrograms of Stamista as a loading dose, followed immediately by an intravenous drip of 250 micrograms per hour. When the interval between two infusions is greater than 3 to 5 minutes, 250 micrograms of Stamista should be reinjected intravenously to ensure continuity of administration. When the bleeding is stopped (usually within 12 to 24 hours), treatment should be continued for 48 to 72 hours to prevent recurrence. For the above cases, the usual treatment duration is 120 hours. For adjunctive treatment of pancreatic fistula, biliary fistula, and intestinal fistula: Continuous intravenous drip at a rate of 250 micrograms per hour should be used until the fistula is closed (2 to 20 days). This treatment can be used as an adjunct to total parenteral nutrition. When the fistula is closed, intravenous drip of silamin should be continued for 1 to 3 days, and then gradually discontinued to prevent rebound effect. Prevention and treatment of complications after pancreatic surgery: At the beginning of the operation, as an adjunctive treatment, silamin is dripped at a rate of 250 micrograms per hour; after the operation, the drip is continued for 5 days. Adjunctive treatment of diabetic ketoacidosis: For patients with ketoacidosis, intravenous drip of silamin at a rate of 100 to 500 micrograms per hour combined with insulin treatment can relieve ketoacidosis within 3 hours and restore blood sugar to normal within 4 hours.

Adverse Reactions

A few cases experienced nausea, dizziness, blushing, etc. When the infusion rate of silamin was higher than 50 micrograms per minute, the patient would experience nausea and vomiting.

Precautions

Patients with confirmed allergies to silafenamide should not use this drug. Pregnant women should avoid using this drug unless there is no other safe alternative.

Drug Interactions

Since silatin can prolong the sleep time caused by cyclohexene barbiturate and intensify the effect of pentylene tetrazole, silatin should not be used simultaneously with such drugs or drugs with the same effect. The incompatibility of this product with other drugs has not been tested, so it should be used alone when injected or dripped.
