Function and Efficacy

This product is a formylated derivative of the reduced form of folic acid, which is the activated form of folic acid in the body (see folic acid). The function of folic acid antagonists such as methotrexate is to bind to dihydrofolate reductase and block the conversion of folic acid to tetrahydrofolate. This product can directly provide the activated form of folic acid in the body, which can rescue the toxic reaction of excessive folic acid antagonists in the body, and is beneficial to the synthesis of thymine nucleotides, DNA, RNA and even proteins. This product can limit the degree of damage of methotrexate to normal cells, and through mutual competition, it can reverse the reaction of methotrexate to bone marrow and gastrointestinal mucosa, but it has no effect on the existing methotrexate neurotoxicity.

Appearance

This product is yellow loose mass or powder.

Indication

It is mainly used as an antidote for folic acid antagonists (such as methotrexate, pyrimethamine or trimethoprim). This product is often used clinically to prevent severe toxic effects caused by overdose or high-dose treatment of methotrexate. When oral folic acid is ineffective, it is also used for megaloblastic anemia caused by stomatitis, diarrhea, malnutrition, pregnancy or infancy, but it is not suitable for vitamin B12 deficiency anemia. In recent years, calcium folinate has been used as an adjuvant treatment for colon and rectal cancer, and combined with fluorouracil, it can prolong survival.

Usage and Dosage

The prescribed dosage and administration time should be strictly followed and should not be changed at will. Additional dosage or discontinuation of the drug must be agreed by the responsible physician. Intramuscular injection as a "rescue" therapy for methotrexate, the dosage of this product is best determined based on the blood drug concentration. The dosage is generally 9-15 mg/m2 per body surface area, once every 6-8 hours, for 2 days, until the serum concentration of methotrexate is below 5×10-8 mol/L. As an antidote for pyrimethamine or trimethoprim, each dose is 9-15 mg intramuscularly, depending on the poisoning situation. For anemia, 1 mg is injected intramuscularly daily. Intravenous injection as an adjuvant treatment for colorectal cancer, combined with fluorouracil; this product is injected intravenously at 200 mg/m2, the injection time is not less than 3 minutes, followed by intravenous injection of 300-400 mg/m2 of fluorouracil, once a day, for 5 consecutive days as a course of treatment, and can be repeated every 4-5 weeks according to toxic reactions to prolong survival. The dosage for children may refer to the adult dosage as appropriate.

Adverse Reactions

Rarely, skin rash may occur. Look for allergic reactions such as hives or asthma.

Precautions

It is contraindicated in pernicious anemia and megaloblastic anemia caused by vitamin B12 deficiency.

Drug Interactions

The use of this product in large doses together with barbiturates, primidone or phenytoin sodium may affect the anti-epileptic effect.
