Heparin Sodium Injection

Because this product has the physical and chemical characteristics of strong negative charge, it can interfere with many links of the blood coagulation process and has anticoagulant effect in vivo and in vitro. Its mechanism of action is relatively complex, mainly through binding with antithrombin III (AT-III), thereby enhancing the latter's inhibitory effect on activated coagulation factors II, IX, X, XI and XII. Its consequences involve preventing platelet aggregation and destruction, hindering the formation of coagulation activating enzymes; preventing prothrombin from turning into thrombin; inhibiting thrombin, thereby preventing fibrinogen from turning into fibrin.

The main ingredients of this product are: heparin sodium. Heparin sodium is a sodium salt of aminoglucosan sulfate extracted and refined from pig intestinal mucosa or cow lung. It belongs to mucopolysaccharide substances. The average molecular weight is 12000.

Used to prevent and treat thrombosis or embolic diseases (such as myocardial infarction, thrombophlebitis, pulmonary embolism, etc.); disseminated intravascular coagulation (DIC) caused by various reasons; also used in hemodialysis, extracorporeal circulation, catheterization, microvascular surgery and other operations and for anticoagulation of certain blood specimens or instruments.

1. Deep subcutaneous injection: 5000-10000 units for the first time, 8000-10000 units every 8 hours or 15000-20000 units every 12 hours; the total amount every 24 hours is about 3000-40000 units, which can generally achieve satisfactory results; 2. Intravenous injection: 5000-10000 units for the first time, or 100 units/kg every 4 hours according to body weight, diluted with sodium chloride injection; 3. Intravenous drip: 20000-40000 units per day, added to 1000ml of sodium chloride injection and continuously dripped. 5000 units can be injected intravenously as the initial dose before drip; 4. Preventive treatment: patients with high risk of thrombosis, mostly used after abdominal surgery to prevent deep vein thrombosis. 5000 units of heparin are injected subcutaneously 2 hours before surgery, but epidural anesthesia should be avoided, and then 5000 units are given every 8-12 hours for about 7 days.

The toxicity is low. The main adverse reaction is that excessive use of the drug can cause spontaneous bleeding, so the coagulation time should be measured before each injection. If severe bleeding occurs after injection, protamine sulfate can be injected intravenously for emergency treatment (1 mg of protamine sulfate can neutralize 150 U of heparin). Occasionally, allergic reactions and thrombocytopenia often occur in the first 5 to 9 days of medication, so the platelet count should be monitored regularly within 1 month of starting treatment. Occasionally, one-time hair loss and diarrhea are seen. It can also cause osteoporosis and spontaneous fractures. Long-term use in patients with poor liver function can cause antithrombin-III depletion and thrombosis tendency.

It is contraindicated for patients who are allergic to heparin, have a tendency to spontaneous bleeding, have slow blood coagulation (such as hemophilia, purpura, thrombocytopenia), have ulcer disease, trauma, postpartum hemorrhage, and severe liver dysfunction.

1. The use of this product in combination with the following drugs may increase the risk of bleeding: ① Coumarin and its derivatives can lead to severe factor IX deficiency and cause bleeding; ② Aspirin and non-steroidal anti-inflammatory analgesics, including mefenamic acid and salicylic acid, can inhibit platelet function and induce gastrointestinal ulcer bleeding; ③ Dipyridamole and dextran may inhibit platelet function; ④ Adrenocortical hormone, adrenocorticotropic hormone, etc. are prone to induce gastrointestinal ulcer bleeding; ⑤ Other drugs such as diuronic acid, tissue plasminogen activator (t-PA), urokinase, and streptokinase. 2. Heparin combined with sodium bicarbonate, sodium lactate and other drugs to correct acidosis can promote the anticoagulant effect of heparin; 3. Heparin mixed with hyaluronidase injection can not only reduce intramuscular injection pain, but also promote heparin absorption. However, heparin can inhibit the activity of hyaluronidase, so the two should be used temporarily in combination, and the drugs should not be left for a long time after mixing; 4. Heparin can interact with insulin receptors, thereby changing the binding and action of insulin. There have been reports of heparin-induced hypoglycemia; 5. The following drugs are incompatible with this product: kanamycin, amikacin, daunorubicin, erythromycin lactobionate, gentamicin sulfate, hydrocortisone sodium succinate, polymyxin B, doxorubicin, tobramycin, vancomycin, cefoperazone, cefoperazone, cephalothin sodium, chloroquine, promethazine, and narcotic analgesics.
