Vinorelbine Tartrate for Injection
Function and Efficacy
The anti-tumor activity of vinorelbine is believed to be mainly inhibiting metaphase mitosis by interfering with tubulin. Similar to other vinca alkaloids, vinorelbine can also interfere with: 1) the metabolism of amino acids, cyclic AMP and glutathione; 2) calmodulin-dependent calcium ion transport ATPase activity; 3) cellular respiration; 4) nucleic acid and lipid biosynthesis. In mouse intact embryo culture, vinorelbine, vincristine and vinblastine inhibited microtubule-forming micromitosis at the same concentration (2uM), including blocking the metaphase division of cells. Vincristine has a depolymerizing effect on axonal microtubules at a concentration of 5uM, while vinblastine and vinorelbine have this effect at 30uM and 40uM. These data indicate that vinorelbine has a relatively selective effect on microtubules in the metaphase of mitosis.
Indication
Non-small cell lung cancer, metastatic breast cancer and refractory lymphoma, ovarian cancer, head and neck tumors.
Usage and Dosage
This product can only be administered intravenously. 1 Monotherapy: The recommended dose is 25-30 mg/m2 each time, 21 days as a cycle, and the drug is administered once on the 1st and 8th days respectively, and 2 to 3 cycles as a course of treatment. 2 Combination therapy: The dosage and administration time vary with the chemotherapy regimen. 3 This product must first be diluted to 50 ml with normal saline, injected intravenously in a short time (6-10 minutes), and then flush the vein with 250-500 ml of normal saline. It is necessary to confirm that the injection needle is in the vein before starting the injection. If the drug leaks out of the vein, it will cause a strong local irritation reaction. Once the drug leaks out, the injection should be stopped immediately, and the remaining drug should be injected into another vein. If the drug accidentally enters the eyes, it should be immediately rinsed with plenty of clean water or isotonic fluid.
Adverse Reactions
Hematologic toxicity: granulocytopenia, moderate anemia; peripheral neurotoxicity: generally limited to decreased deep tendon reflexes, numbness is rare, paresthesia is occasionally seen, and lower limb weakness may occur with long-term use; digestive tract autonomic nervous system: mainly constipation caused by intestinal paralysis, paralytic ileus is rare, and nausea and vomiting are occasionally seen; digestive system: may cause dyspnea or bronchospasm, which may occur within minutes or hours after injection; others: progressive moderate alopecia, mandibular pain, local phlebitis.
Precautions
Contraindicated in patients with a pre-treatment granulocyte count of 1000 cells/mm3.
Drug Interactions
Acute pulmonary reactions have been reported when vinorelbine or other vinca alkaloids are used concomitantly with mitomycin. Although the pharmacokinetics of vinorelbine are not affected by concurrent administration of cisplatin, the incidence of neutropenia is significantly increased when vinorelbine is combined with cisplatin compared with vinorelbine alone. Patients receiving vinorelbine and docetaxel should be monitored for symptoms of neuropathy during or after concurrent use. Vinorelbine may increase sensitivity to radiation effects in patients who have previously received radiation therapy.
