Function and Efficacy

Piperacillin is a semi-synthetic penicillin antibiotic with a broad-spectrum antibacterial effect. Piperacillin has good antibacterial effects on Enterobacteriaceae such as Escherichia coli, Proteus, Serratia, Klebsiella, Enterobacter, Citrobacter, Salmonella and Shigella, as well as other Gram-negative bacteria such as Pseudomonas aeruginosa, Acinetobacter, Haemophilus influenzae and Neisseria. This product also has certain antibacterial activity against Enterococci, Group A and Group B hemolytic streptococci, Streptococcus pneumoniae and Staphylococci that do not produce penicillinase. Many anaerobic bacteria, including Bacteroides fragilis and Clostridium, are also sensitive to piperacillin. The mechanism of action of piperacillin is to exert a bactericidal effect by inhibiting bacterial cell wall synthesis.

Ingredients

The main ingredient of this product is piperacillin sodium, and its chemical name is (2S,5R,6R)-3.3-dimethyl-6-[(4-ethyl-2,3-dioxo-1-piperazinecarboxamido)phenylacetylamino]-7-oxo-4-thia-1-azabicyclo[3.2.0]heptane-2-carboxylic acid sodium salt.

Appearance

This product is white or off-white powder or loose lumps; odorless; highly hygroscopic.

Indication

It is suitable for sepsis caused by sensitive Enterobacteriaceae, Pseudomonas aeruginosa, and Acinetobacter, upper urinary tract and complicated urinary tract infections, respiratory tract infections, biliary tract infections, abdominal infections, pelvic infections, and skin and soft tissue infections. Piperacillin combined with aminoglycosides can also be used for infections in immunocompromised patients with neutropenia.

Usage and Dosage

This product can be used for intravenous drip and intravenous injection. For moderate infection in adults, take 8g a day, divided into 2 intravenous drips; for severe infection, take 3-4g once, intravenously drip or inject every 4-6 hours. The total daily dose should not exceed 24g. The dose for infants and children under 12 years old is 100-200mg/kg per day according to body weight. For newborns weighing less than 2kg, take 50mg/kg intravenously every 12 hours in the first week after birth; 50mg/kg every 8 hours from the second week onwards. For newborns weighing more than 2kg, take 50mg/kg intravenously every 8 hours in the first week after birth; 50mg/kg every 6 hours for those weighing more than 1 week.

Adverse Reactions

1. Allergic reactions: Allergic reactions to penicillins are common, including urticaria and other types of rashes, leukopenia, interstitial nephritis, asthma attacks and serum sickness reactions. In severe cases, anaphylactic shock is occasionally seen. Once anaphylactic shock occurs, emergency treatment must be given on the spot, including maintaining airway patency, oxygen inhalation, and administration of epinephrine, glucocorticoids and other treatment measures. 2. Local symptoms: pain at the local injection site, thrombophlebitis, etc. 3. Gastrointestinal symptoms: diarrhea, loose stools, nausea, vomiting, etc.; pseudomembranous colitis is rare. 4. Cholestatic jaundice may occur in some patients. 5. Central nervous system symptoms: headache, dizziness and fatigue, etc. 6. When patients with impaired renal function use large doses, penicillin encephalopathy may occur due to increased cerebrospinal fluid concentration, so the dose should be adjusted according to renal function at this time. 7. Others: Candida superinfection, bleeding, etc.

Precautions

It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.

Drug Interactions

1 In vitro, this product has a synergistic antibacterial effect against Pseudomonas aeruginosa and some Enterobacteriaceae when used in combination with aminoglycosides (amikacin, gentamicin or tobramycin). 2 This product is used in combination with cefoxitin, because the latter can induce bacteria to produce b-lactamase and has an antagonistic effect on Pseudomonas aeruginosa, Serratia, Proteus and Enterobacter. 3 When used in combination with anticoagulants such as heparin, coumarin, indandione and non-steroidal anti-inflammatory analgesics, the risk of bleeding may be increased, and when used in combination with suppository solvents, severe bleeding may occur. 4 This product cannot be infused in the same bottle with aminoglycoside antibiotics, otherwise the antibacterial activity of both will be weakened.
