Function and Efficacy

This product is an antibiotic drug. Except for enterococci, methylphenidate-resistant and penicillin-resistant Staphylococcus aureus, this product has good antibacterial activity against other Gram-bacterial cocci. Pneumococci and lytic streptococci are highly sensitive to this product. Diphtheria bacillus, anthrax bacillus, Listeria and Clostridium are sensitive to this drug. This product has stronger antibacterial activity against Escherichia coli, Proteus mirabilis and Klebsiella pneumoniae than cephalothin and cefotaxime, but its antibacterial effect against high-content bacteria is weaker than the latter two. Salmonella typhi and Shigella are sensitive to this drug, while other Enterobacteriaceae, Acinetobacter and Pseudomonas aeruginosa and Bacteroides fragilis are resistant. Neisseria is quite sensitive to this product, and influenza bacillus is only moderately sensitive.

Indication

It is suitable for pneumonia and other lower respiratory tract infections, urinary tract infections, meningitis, sepsis, abdominal infections, pelvic infections, skin and soft tissue infections, reproductive tract infections, bone and joint infections caused by sensitive bacteria. Cefotaxime can be used as a drug of choice for infantile meningitis.

Usage and Dosage

Adults: 2-6g per day, divided into 2-3 times of intravenous injection or intravenous drip; patients with severe infection: 2-3g every 6-8 hours, and the maximum daily dose shall not exceed 12g. For the treatment of uncomplicated pneumococcal pneumonia or acute urinary tract infection, 1g every 12 hours. For newborns aged 7 days or less, 50mg/kg every 12 hours, and for those born more than 7 days old, 50mg/kg every 8 hours. The dose for treating meningitis patients can be increased to 75mg/kg every 6 hours, all administered intravenously. Patients with severe renal impairment should reduce the dose appropriately when using this product. When the serum creatinine value exceeds 424?mol/L (4.8mg) or the creatinine clearance rate is less than 20ml/min, the maintenance dose of this product should be halved; when the serum creatinine exceeds 751?mol/L (8.5mg), the maintenance dose is 1/4 of the normal amount. For patients who need hemodialysis, 0.5-2g per day. However, one dose should be added after dialysis.

Adverse Reactions

The thrombophlebitis and pain at the intramuscular injection site that occur with intravenous injection are milder and less frequent than those with cephalothin. The incidence of drug eruption is 1.1%. The incidence of eosinophilia is 1.7%. Allergic reactions manifested by drug fever alone have also been reported occasionally. Some patients may experience temporary elevations in serum transaminases and alkaline phosphatase.

Precautions

This product is contraindicated for patients who are allergic to cephalosporins or have a history of anaphylactic shock or immediate reaction to penicillin. It should be used with caution in lactating women, patients with a history of gastrointestinal diseases, patients with liver damage and renal dysfunction, and patients with severe penicillin allergy. The dose should be reduced by 1/4-1/2 for patients with severe renal failure, and the product should be prepared and used immediately.

Special Population Medication

Precautions during pregnancy and lactation: Use with caution in lactating women.

Drug Interactions

1. It has a synergistic effect with gentamicin or tobramycin against Pseudomonas aeruginosa; it has a synergistic effect with amikacin against Escherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa. 2. When used in combination with aminoglycoside antibiotics, renal function should be followed up during medication. 3. When large doses of cefotaxime are used in combination with strong diuretics, attention should be paid to changes in renal function. 4. Cefotaxime can be diluted with sodium chloride injection or glucose solution, but cannot be mixed with sodium bicarbonate solution. 5. When used in combination with azlocillin or mezlocillin, the total clearance rate of this product can be reduced. If the two are used in combination, the dose should be appropriately reduced.
