Function and Efficacy

Nimodipine is a Ca2 channel blocker. Under normal circumstances, the contraction of smooth muscle depends on the entry of Ca2 into the cell, causing the depolarization of the transmembrane current. Nimodipine achieves the purpose of relieving vasospasm by effectively preventing Ca2 from entering the cell and inhibiting the contraction of smooth muscle. Animal experiments have shown that the effect of nimodipine on cerebral arteries is much stronger than that on arteries in other parts of the body, and because it has a high lipophilicity, it is easy to pass through the blood-brain barrier. When used for the treatment of subarachnoid hemorrhage, the concentration in cerebrospinal fluid can reach 12.5ng/ml. It can be inferred that it can be used clinically to prevent vasospasm after subarachnoid hemorrhage, but the mechanism of action of this drug in human application is still unclear. In addition, it also has the effect of protecting and promoting memory and promoting intellectual recovery. Therefore, it can selectively act on cerebral vascular smooth muscle, dilate cerebral blood vessels, increase cerebral blood flow, and significantly reduce ischemic brain damage caused by vasospasm.

Indication

It is suitable for improving blood circulation in the recovery period of cerebral vasospasm after subarachnoid hemorrhage of various reasons and acute cerebrovascular disease.

Usage and Dosage

For subarachnoid hemorrhage, intravenous drip of this product should be started as soon as possible, 25 mg per dose, at a rate of 0.5 μg/(kg/min), and blood pressure should be monitored. It is best if the blood pressure does not drop or drops slightly. Then switch to oral administration, 30-60 mg each time, 3 times a day. For acute cerebral ischemia, the drug should be dripped as soon as possible, with the same dosage and speed as above, and then switched to oral administration.

Adverse Reactions

A large number of clinical practices have shown that about 11.2% of patients with subarachnoid hemorrhage experience adverse reactions when treated with nimodipine. The most common adverse reactions are: 1. Decreased blood pressure, the degree of which is related to the drug dose. 2. Hepatitis. 3. Skin tingling. 4. Gastrointestinal bleeding. 5. Thrombocytopenia. 6. Occasionally transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc. In addition, some patients may experience increased levels of alkaline phosphatase (ALP), lactate dehydrogenase (LDH), increased blood sugar, and increased platelet count in some individuals.

Precautions

Patients with severe liver damage are contraindicated. It is not recommended to take nimodipine and anti-epileptic drugs at the same time.

Drug Interactions

1. When used in combination with other calcium ion antagonists that act on the cardiovascular system, it can increase the effectiveness of other calcium ion antagonists. 2. When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the fact that cimetidine inhibits cytochrome P450 in the liver and blocks the metabolism of nimodipine.
