Nimodipine Injection

Nimodipine is a Ca2 channel blocker. Under normal circumstances, the contraction of smooth muscle depends on Ca2 entering the cell, causing depolarization of the transmembrane current. Nimodipine achieves the purpose of relieving vasospasm by effectively preventing Ca2 from entering the cell and inhibiting smooth muscle contraction. Animal experiments have shown that the effect of nimodipine on cerebral arteries is much stronger than that on arteries in other parts of the body, and because it has a high lipophilicity, it can easily pass through the blood-brain barrier. When used to treat subarachnoid hemorrhage, the concentration in cerebrospinal fluid can reach 12.5ng/ml. It can be inferred that it can be used clinically to prevent vasospasm after subarachnoid hemorrhage, but the mechanism of action of this drug in humans is still unclear. In addition, it also has the effect of protecting and promoting memory and promoting intellectual recovery.

This product is an almost colorless or slightly yellowish green clear liquid.

It is suitable for improving blood circulation in the recovery period of cerebral vasospasm after subarachnoid hemorrhage of various reasons and acute cerebrovascular disease.

For subarachnoid hemorrhage, intravenous drip of this product should be started as soon as possible, 25 mg per dose, at a rate of 0.5 μg/(kg·min), and blood pressure should be monitored. It is best if the blood pressure does not drop or drops slightly. Then switch to oral administration, 30~60 mg each time, 3 times a day. For acute cerebral ischemia, the drug should be dripped as soon as possible, with the same dosage and speed as above, and then switched to oral administration.

A large number of clinical practices have shown that when patients with subarachnoid hemorrhage are treated with nimodipine, about 11.2% of patients experience adverse reactions. The most common adverse reactions are: (1) Decreased blood pressure, the degree of which is related to the drug dose. (2) Hepatitis. (3) Skin tingling. (4) Gastrointestinal bleeding. (5) Thrombocytopenia. (6) Occasionally, transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc. In addition, after oral administration of nimodipine, some patients may experience elevated levels of alkaline phosphatase (ALP), lactate dehydrogenase (LDH), and AKP, and blood

Patients with severe liver damage are contraindicated. It is not recommended to take nimodipine and anti-epileptic drugs at the same time.

Precautions during pregnancy and lactation: The reproductive toxicity of nimodipine injection has not been studied. When using this product in pregnant women, the pros and cons must be carefully weighed according to the patient's condition. Use with caution in lactating women.

(1) When used in combination with other calcium ion antagonists that act on the cardiovascular system, it can increase the effectiveness of other calcium ion antagonists. (2) When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the inhibition of nimodipine metabolism by cimetidine in the liver cytochrome P450.
