Fosfomycin Calcium Trimethoprim Capsules
Function and Efficacy
Fosfomycin calcium in this product inhibits the early synthesis of bacterial cell walls, which inhibits bacterial cell wall synthesis and leads to their death. The mechanism of action of trimethoprim (TMP) in this product is to interfere with bacterial folic acid metabolism, selectively inhibit the activity of bacterial dihydrofolate reductase, so that dihydrofolate cannot be reduced to tetrahydrofolate, and prevent the synthesis of bacterial nucleic acids and proteins. This product has a wide antibacterial spectrum and has a certain antibacterial effect on most Gram-positive and Gram-negative bacteria. The antibacterial spectrum includes Staphylococcus aureus, Escherichia coli, Shigella, Serratia, Shigella, Pseudomonas aeruginosa, Klebsiella pneumoniae and Enterobacter aerogenes.

Indication
Used for various infectious diseases caused by Gram-positive and Gram-negative bacteria.
Usage and Dosage
Oral administration: 2 to 4 capsules each time (each capsule contains 0.125 g of fosfomycin), 4 times a day, reduce the dosage for children.
Adverse Reactions
1. Mainly mild gastrointestinal reactions, such as nausea, decreased appetite, upper abdominal discomfort, loose stools or mild diarrhea, etc. Generally, it does not affect the continued use of the drug, but occasionally rash, eosinophilia, elevated alanine aminotransferase, etc. may occur. 2. The interference of TMP in this product with folic acid metabolism can produce adverse reactions in the blood system, such as leukopenia, thrombocytopenia or methemoglobin anemia. Generally, leukopenia and thrombocytopenia are mild, and recovery is expected if the drug is stopped in time. Folic acid preparations can also be added for treatment. 3. Allergic reactions: Itching and rash may occur, and occasionally severe exudative erythema multiforme may occur. 4. Aseptic meningitis may occasionally occur, with symptoms such as headache, stiff neck, and nausea.
Precautions
1. It is contraindicated for those who are allergic to this product. 2. It is contraindicated for patients with severe liver and kidney diseases. 3. It is contraindicated for patients with blood diseases (such as leukopenia, thrombocytopenia, purpura, etc.). 4. It is contraindicated for pregnant and lactating women. 5. It is contraindicated for newborns and premature infants.
Drug Interactions
1. It has synergistic effect with β-lactams against Staphylococcus aureus (including methicillin-resistant Staphylococcus aureus) and Pseudomonas aeruginosa, and delays the development of drug resistance. 2. It has synergistic effect with aminoglycosides, and delays the development of drug resistance. 3. There is no cross-resistance with other antibiotics. 4. The chance of leukopenia and thrombocytopenia increases when bone marrow suppressants are used in combination with TMP. 5. When dapsone is used in combination with this product, the blood concentration of both drugs can be increased. The increase of the former can increase and aggravate adverse reactions, especially the occurrence of methemoglobinemia. 6. This product should not be used simultaneously with anti-tumor drugs and 2,4-diaminopyrimidine drugs, nor should it be used between courses of treatment with other folic acid antagonists, because it may cause bone marrow aplasia or megaloblastic anemia. 7. When used in combination with rifampicin, TMP clearance can be significantly increased and serum half-life can be shortened. 8. When used in combination with cyclosporine, renal toxicity can be increased. 9. This product can interfere with the hepatic metabolism of phenytoin, increase the half-life (t1/2) of phenytoin by 50%, and reduce its clearance rate. 10. When used in combination with procainamide, it can reduce the renal clearance of procainamide, resulting in an increase in the blood concentration of procainamide and its metabolite NAPA. 11. When used in combination with warfarin, TMP can inhibit the metabolism of the drug and enhance its anticoagulant effect.
