Function and Efficacy

Theophylline has a direct relaxant effect on the smooth muscle of the respiratory tract, and its effect is partly due to the release of endogenous adrenaline and norepinephrine. In addition, theophylline is a purine receptor blocker, which can counteract the contraction of adenine and other substances on the respiratory tract. Theophylline can enhance the contraction of the diaphragm, especially when the diaphragm is weak, so it is beneficial to improve respiratory function. Methylephedrine hydrochloride can directly stimulate adrenaline receptors, and can also indirectly stimulate adrenaline receptors by prompting adrenergic nerve endings to release norepinephrine, and has an stimulating effect on both alpha and beta receptors. It can dilate the bronchi and contract local blood vessels to eliminate congestion and swelling of the nasopharyngeal mucosa, relieve nasal congestion symptoms, and has a longer duration of action; strengthen myocardial contractility, increase cardiac output, and make venous return to the heart sufficient; it has a stronger central nervous system excitation effect than adrenaline.

Appearance

This product is light brown flakes.

Indication

Used for chronic bronchitis, bronchial asthma, and protracted acute bronchitis.

Usage and Dosage

Oral administration: 1 tablet at a time, 3 times a day. For children or elderly patients, reduce the dosage or follow the doctor's advice.

Adverse Reactions

1. The toxicity of theophylline often occurs when the serum concentration is 15-20 mu; g/ml, especially at the beginning of treatment. In the early stage, it is common to have bad heart, vomiting, irritability, insomnia, etc. When the serum concentration exceeds 20 mu; g/ml, tachycardia and arrhythmia may occur. When the theophylline in the serum exceeds 40 mu; g/ml, fever, dehydration, convulsions and other symptoms may occur. In severe cases, respiratory and cardiac arrest may even lead to death. 2. It may cause urination difficulties in patients with prostatic hypertrophy.

Precautions

1. Patients who are allergic to any of the ingredients in this product are prohibited from using this product. 2. Patients with peptic ulcers and uncontrolled convulsive diseases are prohibited from using this product. 3. Patients with hyperthyroidism, hypertension, arteriosclerosis, and angina pectoris are prohibited from using this product.

Special Population Medication

Precautions for children: Children should reduce the dosage as appropriate or follow the doctor's advice. Precautions for pregnancy and lactation: Theophylline can pass through the placental barrier and can also be secreted into breast milk. Pregnant and lactating women should use it with caution. Precautions for the elderly: Elderly patients, especially those over 55 years old, should use it with caution.

Drug Interactions

1. Theophylline (1) Diltiazem and verapamil can interfere with the metabolism of theophylline in the liver. When used in combination with this product, the blood concentration and toxicity of this product will be increased. (2) Cimetidine can reduce the liver clearance rate of this product. When used in combination, it can increase the serum concentration and (or) toxicity of theophylline. (3) Certain antibacterial drugs, such as erythromycin, roxithromycin, and clarithromycin of the macrolide class, and enoxacin, ciprofloxacin, ofloxacin, clindamycin, and lincomycin of the quinolone class, can reduce the clearance rate of theophylline and increase its blood concentration, especially erythromycin and enoxacin. When theophylline is used in combination with the above drugs, the dosage should be appropriately reduced. (4) Phenobarbital, phenytoin, and rifampicin can induce liver enzymes and increase the liver clearance rate of theophylline; theophylline also interferes with the absorption of phenytoin. The plasma concentrations of both decrease. The dosage should be adjusted when used in combination. (5) When used in combination with lithium salts, it can increase the renal excretion of lithium and affect the effect of lithium salts. (6) When used in combination with mexiletine, the clearance rate of theophylline can be reduced and the concentration of theophylline in plasma can be increased, requiring dosage adjustment. (7) When used in combination with caffeine or other xanthine drugs, their effects and toxicity can be increased. 2. Methylephedrine hydrochloride (1) Urine alkalinizing agents, such as antacids, carbonates of calcium or magnesium, citrates, sodium bicarbonate, etc., affect the excretion of this product in urine, increase the half-life of this product, and prolong the duration of action. In particular, if the urine remains alkaline for several days or longer, most patients will suffer from ephedrine poisoning, and the dosage of this product should be adjusted. (2) When used in combination with digitalis glycosides, it can cause arrhythmias.
