Function and Efficacy

Cefuroxime sodium is a second-generation broad-spectrum, long-acting cephalosporin antibiotic that produces antibacterial activity by inhibiting bacterial cell wall synthesis. It has antibacterial effects on both Gram-positive and Gram-negative bacteria and some anaerobic bacteria. It is stable to most β-lactamases. In vitro and clinical trials have confirmed that cefuroxime is effective against the following microorganisms: Gram-positive aerobic bacteria: Staphylococcus aureus (including strains that produce or do not produce β-lactamases), Staphylococcus epidermidis (also resistant to methicillin-resistant Staphylococci), Streptococcus pneumoniae, Streptococcus pyogenes (Group A β-hemolytic streptococci), Streptococcus agalactiae (Group B streptococci). Gram-negative aerobic bacteria: Escherichia coli, Klebsiella pneumoniae, Providencia rettgeri, Morganella morganii, Proteus vulgaris, Proteus mirabilis, Haemophilus influenzae (including ampicillin-sensitive and resistant strains). In vitro antibacterial data show that cefuroxime is effective against the following microorganisms, but its clinical effectiveness has not yet been determined: Gram-negative aerobic bacteria: Moraxella, Klebsiella, Enterobacter aerogenes, Neisseria gonorrhoeae (including penicillin-sensitive and -resistant bacteria), Citrobacter freundii, C. diversus. Gram-positive anaerobic bacteria: Clostridium perfringens, anaerobic Streptococcus, P. prevotii, Propionibacterium acnes. Gram-negative anaerobic bacteria: Fusobacterium nucleatum. Cefuroxime is ineffective against Pseudomonas, Serratia, Enterococcus, Acinetobacter, and Bacteroides fragilis in vitro.

Appearance

This product is white to off-white crystalline powder, odorless.

Indication

It is used for infections caused by the following sensitive bacteria: lower respiratory tract infection, urinary tract infection, sepsis, skin and soft tissue infection, bone and joint infection, and can also be used to prevent infection during surgery. A single dose injection of 1g of cefuroxime before surgery can reduce the incidence of postoperative infection due to contamination or potential contamination during surgery. The use of cefuroxime during cesarean section (after cutting the umbilical cord) can reduce the incidence of certain postoperative infections.

Usage and Dosage

Intramuscular injection, intravenous injection or intravenous drip: Adults generally take 0.5g-1.0g per day, divided into 1-2 times; for severe and refractory infections, the dosage for adults can be increased to 2g per day; for children, the dosage can be increased to 25mg-150mg/kg per day, depending on the severity of infection.

Adverse Reactions

Cefuroxime sodium is generally well tolerated, with the most common adverse reaction being pain during intramuscular injection. Adverse reactions with an incidence rate greater than 1% include: injection site reactions: pain and discomfort, burning sensation at the intravenous injection site, phlebitis; blood system: increased platelets, increased eosinophils; abnormal liver function laboratory tests: increased alkaline phosphatase, increased serum transaminases (ALT, AST), increased lactate dehydrogenase (LDH), increased glutamine transpeptidase (GGTP); adverse reactions with an incidence rate less than 1% include: allergic reactions: fever, rash, urticaria, itching, erythema, myalgia, allergic reactions, Stevens-Johnson syndrome, etc.; gastrointestinal tract: nausea, vomiting, diarrhea, pseudomembranous colitis; blood system: leukopenia, neutropenia, thrombocytopenia, hemolytic anemia, positive Coombs’ test; renal toxicity: occasional increase in blood urea nitrogen and creatinine values, interstitial nephritis, and rare reports of acute renal failure; central nervous system: convulsions (in high doses or renal dysfunction), headache, and mental tension; neuromusculoskeletal: joint pain. Others: candidiasis.

Precautions

It is contraindicated in patients who are allergic to cephalosporins.

Drug Interactions

1. Toxic kidney damage has been reported when used in combination with other cephalosporins and aminoglycosides. It cannot be administered in the same injection container as aminoglycosides. 2. When used in combination with probenecid, it can slow renal excretion, increase blood drug concentration levels, and cause toxicity. 3. When used in combination with potent diuretics, it can lead to increased renal toxicity. 4. Tetracycline, erythromycin and chloramphenicol can reduce the effect of this drug. 5. When used with alcohol, this drug may cause disorder reactions. 6. Cefuroxime can reduce the effect of oral contraceptives. Other effective contraceptive methods should be used. 7. Interference with laboratory test values, increased false positives in Coombs tests.
