Function and Efficacy

This product is effective against a variety of Gram-positive and Gram-negative bacteria. Positive bacteria include Staphylococcus aureus, Staphylococcus epidermidis (including resistant and some methicillin-resistant strains), Pneumococcus, Streptococcus faecalis, and other Streptococcus species; negative bacteria include Escherichia coli, Haemophilus influenzae, Branhamella, Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, etc.; anaerobic bacteria include Bacillus fragilis. Ampicillin sodium is the bactericidal component in this product. It acts on the active reproduction stage of bacteria by inhibiting the biosynthesis of cell wall mucopeptides to achieve bactericidal effects. Probenecid is a benzoic acid derivative. It competes with ampicillin for the same active transport carrier in the renal tubules, inhibiting the latter's excretion from the renal tubules, increasing the blood concentration of ampicillin and prolonging its blood elimination half-life (t1/2?).

Appearance

The content of this product is white or off-white powder with a slightly bitter taste.

Indication

This product is a broad-spectrum semi-synthetic penicillin with extremely low toxicity. The antibacterial spectrum is similar to that of penicillin, and it has a strong antibacterial effect on grass-green streptococci, hemolytic streptococci, and enterococci. Corynebacterium diphtheriae, Bacillus anthracis, Gram-positive anaerobic cocci and bacilli are sensitive to this product; meningococci, Bordetella pertussis, and Brucella are also very sensitive. Escherichia coli, Salmonella, Shigella dysenteriae, Proteus, Bacillus influenzae, Pneumococcus, and Neisseria gonorrhoeae all have varying degrees of drug resistance because some strains produce β-lactamase. Among them, the resistance rate of Escherichia coli is as high as 50-60%. Enterobacter bacteria, Pseudomonas aeruginosa, and anaerobic Gram-negative bacilli are resistant to this product. Usage and Dosage

Oral administration on an empty stomach. Common dosage: 2 capsules per time for adults, 1 capsule for children aged 6-12 years. 2/3 capsules for children aged 2-6 years, 3 times a day. Children under 1 year old, 0.05-0.15g per kg of body weight per day, divided into 3-4 times. Or follow the doctor's advice.

Adverse Reactions

1. The adverse reactions of this product are similar to those of ampicillin, with allergic reactions being more common. Rash is the most common reaction, which often occurs 5 days after medication and is in the form of urticaria or maculopapular rash; interstitial nephritis may also occur; anaphylactic shock is occasionally seen. 2. Gastrointestinal reactions such as glossitis, gastritis, nausea, vomiting, enteritis, diarrhea, and mild abdominal pain are also common. 3. Granulocytopenia and thrombocytopenia are occasionally seen in patients taking ampicillin. Antibiotic-related enteritis is rare, and a small number of patients have elevated serum alanine aminotransferase (ALT).

Precautions

1. Patients who are allergic to penicillins and cephalosporins or have a positive penicillin skin test are prohibited from using this product. 2. Patients with uric acid kidney stones, acute gout attacks, and active gastrointestinal ulcers are prohibited from using this product. Children under 32 years old are prohibited from using this product.

Special Population Medication

Precautions for children: Children under 2 years old are prohibited from taking this medicine. Precautions for pregnancy and lactation: There are no strict controlled trials for the use of ampicillin in pregnant women, so pregnant women should only use this medicine when it is necessary. A small amount of ampicillin can be secreted from breast milk, so breastfeeding should be suspended when taking this medicine. Precautions for the elderly: Due to the decline of physiological renal function, the dosage should be appropriately reduced.

Drug Interactions

Gentamycin: Penicillin, carbenicillin, ampicillin and other penicillin antibiotics can inactivate gentamicin. Glucose solution (pH 3.2-5.5): Ampicillin is easily inactivated in acidic media, reducing its efficacy. Vitamin C: Can inactivate or reduce the efficacy of ampicillin. Tetracycline: Can reduce the efficacy of penicillin in treating pneumonia, meningitis and scarlet fever. Dietary fiber: Can reduce the absorption of oral ampicillin. Balanced solution: Its lactate root can promote the hydrolysis and efficacy reduction of ampicillin sodium (reduced to 75% in 30 minutes). Indomethacin: Can prolong the half-life of penicillin and increase the blood drug concentration. Aspirin: Can increase the blood concentration of penicillin and prolong the half-life of penicillin from 44.5 minutes to 72.4 minutes (competition for renal tubular secretion).
