Function and Efficacy

The main pharmacological effect of all nitrates is the same, that is, relaxing vascular smooth muscle. Organic nitrates release nitric oxide (NO) in the body. NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris. No carcinogenic and mutagenic phenomena were observed in animal experiments.

Indication

Vasodilators are drugs used primarily to treat or prevent attacks of angina pectoris.

Usage and Dosage

When angina pectoris attacks, spray 1-2 times on the sublingual mucosa of the oral cavity, which is equivalent to 0.5-1 mg of nitroglycerin. When using, first remove the spray cap, put the cover on the spray head, turn the bottle upside down, aim the cover at the sublingual mucosa of the oral cavity, press the valve, and the liquid medicine will be sprayed into the oral cavity in the form of mist.

Adverse Reactions

In the early stage of medication, nitrate-induced vasodilatory headache may occur, as well as facial flushing, dry mouth, dizziness, orthostatic hypotension and reflex tachycardia. Occasionally, blood pressure may drop significantly, bradycardia, angina pectoris may worsen and syncope may occur. Some patients may experience numbness in the oral mucosa.

Precautions

It is contraindicated for patients with glaucoma, shock or hypotension.

Drug Interactions

1. Use of this drug may cause hypotension when drinking moderately or excessively. 2. Combination with antihypertensive drugs or vasodilators may enhance the postural hypotensive effect of nitrates. 3. Aspirin may reduce the clearance of sublingual nitroglycerin and enhance its hemodynamic effects. 4. The use of long-acting nitrates may reduce the therapeutic effect of sublingual medications. 5. Sildenafil citrate (Viagra) enhances the antihypertensive effect of organic nitrates. 6. The efficacy may be weakened when used in combination with acetylcholine, histamine and sympathomimetic amines.
