Function and Efficacy

This product is a slow channel blocker. It combines with the slow channel in phase 2 of the action potential to prevent calcium ions from entering the cell. It mainly acts selectively on vascular smooth muscle, dilates coronary arteries and collateral vessels, increases coronary flow, thereby improving blood flow in the ischemic area and limiting the expansion of the scope of myocardial infarction; it can dilate peripheral blood vessels, lower blood pressure, reduce cardiac load, reduce cardiac work and oxygen consumption, and improve myocardial energy metabolism, but the negative inotropic effect is slight at therapeutic doses. Diltiazem hydrochloride has a non-competitive antagonistic effect on vasoactive substances such as catecholamines, acetylcholine, and histamine. In terms of electrophysiology, at therapeutic doses, diltiazem hydrochloride can prolong the effective refractory period and relative refractory period of the atrioventricular node, and prolong the P-R interval and A-H interval.

Appearance

This product is a hard capsule, and the contents are white or off-white spherical pellets.

Indication

Used for hypertension and angina pectoris caused by coronary heart disease.

Usage and Dosage

Start with a small dose and adjust the dose according to the condition. The oral dosage is 90mg or 120mg, 1 tablet 1 to 2 times a day; the dosage is 180mg or 240mg, 1 tablet once a day.

Adverse Reactions

Symptoms may include headache, dizziness, facial flushing, edema, stomach discomfort, rash, fatigue, bradycardia, etc.; occasionally, atrioventricular block, sinoatrial arrest, hair loss, and mild abnormal liver function, etc. Symptoms may return to normal after stopping the drug.

Precautions

It is contraindicated in patients with conduction block of degree II or above, sick sinus syndrome, hypotension, acute myocardial infarction and pulmonary congestion, pregnant women and patients allergic to this product.

Special Population Medication

Precautions for children: The safety and effectiveness of this product for children have not been determined. Precautions for pregnancy and lactation: Pregnant women are prohibited from using this product. Precautions for the elderly: No clinical data on the use of this product for the elderly have been found, but it is recommended that elderly patients can start using this product at half the normal dose.

Drug Interactions

1 beta-receptor blockers: Studies have shown that diltiazem hydrochloride is well tolerated when used in combination with beta-receptor blockers, but there is insufficient data in patients with left ventricular dysfunction and conduction dysfunction. This product can increase the bioavailability of propranolol by nearly 50%, so the dose of propranolol needs to be adjusted when starting or stopping the combination of the two drugs. 2 Cimetidine: Due to the inhibition of cytochrome P450 oxidase, the first-pass metabolism of this product can be affected, which can significantly increase the peak blood concentration of this product and the area under the drug-time curve. Ranitidine only slightly increases the blood concentration of this product. 3 Digoxin: There are reports that this product can increase the blood concentration of digoxin by 20%, but there are also reports that it has no effect. Although the results are contradictory, the blood concentration of digoxin should be monitored when starting, adjusting and stopping treatment with this product to avoid excessive or insufficient digoxin. 4 Anesthetics: They have inhibitory effects on myocardial contraction, conduction, and automaticity, and have vasodilation effects, which can produce synergistic effects with this product. Therefore, the dose must be carefully adjusted when the two drugs are used together. 5 Benzodiazepines: This product can significantly increase the peak plasma concentration of triazolam and midazolam and prolong their elimination half-life. 6 Carbamazepine: After this product is used in combination with carbamazepine, the blood concentration of carbamazepine can increase by 40-72% in some cases, leading to toxicity. 7 Cyclosporine: It was found in heart and kidney transplant patients that when this product is used in combination with cyclosporine, the dose of cyclosporine should be reduced by 15-48% to ensure that the drug concentration of cyclosporine is the same as before the combination of this product. The plasma drug concentration of cyclosporine should be monitored when the two are used in combination, especially when starting, adjusting the dose, or stopping the use of this product. The effect of cyclosporine on the plasma drug concentration of this product is unknown. 8 Rifampicin: After this product is used in combination with rifampicin, the plasma drug concentration and efficacy of this product can be significantly reduced.
