Indication

For the treatment of essential hypertension. Mikaso fixed-dose combination (telmisartan 80 mg/hydrochlorothiazide 12.5 mg) is used to treat patients whose blood pressure cannot be adequately controlled with telmisartan alone.

Usage and Dosage

In order to reduce dose-independent side effects, combination therapy is usually only used when the patient has not achieved the desired therapeutic effect with monotherapy. When a patient's blood pressure is not effectively controlled with telmisartan alone at 80 mg, or when a patient's blood pressure is not controlled with hydrochlorothiazide alone at 25 mg once daily, or is controlled but hypokalemia occurs, consider switching to telmisartan hydrochlorothiazide capsules (i.e. 80 mg telmisartan/12.5 mg hydrochlorothiazide), 1 capsule per day, 1 time a day. If necessary, the maximum dose can be increased to 160/25 mg. Patients with hypovolemia should use telmisartan after blood volume correction or under close medical supervision. Adverse Reactions

Adverse events reported with an incidence of less than 2% but significantly higher than placebo in patients treated with telmisartan/hydrochlorothiazide included back pain, dyspepsia, vomiting, tachycardia, hypokalemia, bronchitis, pharyngitis, rash, postural hypotension, and abdominal pain. Adverse events with an incidence of 2% or slightly higher in the treatment group but also common in the placebo group included pain, headache, cough, and urinary tract infection. There were no differences in the incidence of adverse events among different sexes, ages, or races.

Precautions

1. Patients who are allergic to any ingredient in the drug. 2. Patients with anuria and patients who are allergic to other thiamine derivatives. 3. Patients who are allergic to other angiotensin receptor antagonists. 4. Patients with biliary obstructive diseases. 5. Patients with severe liver dysfunction. 6. Patients with severe renal dysfunction. 7. Pregnant and lactating women.

Special Population Medication

Precautions for children: The safety and effectiveness of the drug for children have not been determined. Therefore, this product should be used with caution in children. Precautions for pregnancy and lactation: When used by pregnant women during the second to third trimester of pregnancy, the drug will directly act on the renin-angiotensin system, which may cause damage to the developing fetus or even death. Therefore, when pregnancy is detected, the use of Telmisartan Hydrochlorothiazide Capsules should be terminated as soon as possible. Animal studies have shown that Telmisartan can appear in the milk of lactating rats. It is not clear whether Telmisartan is excreted through human milk. Considering the potential side effects on infants, this product should be prohibited for lactating women. Precautions for the elderly: Current clinical studies have shown that there is no difference in the effectiveness and safety of this product in elderly cases and young cases, but it cannot be ruled out that some elderly individuals are more sensitive to the drug.

Drug Interactions

1. Telmisartan (1) The simultaneous use of telmisartan and digoxin can increase the peak and trough concentrations of digoxin by 49% and 20%, respectively. Therefore, it is recommended to monitor digoxin levels when starting, adjusting or stopping the use of telmisartan to avoid insufficient or excessive digitalization. (2) Telmisartan will slightly reduce the plasma trough concentration of warfarin. (3) No clinically significant interactions were found when telmisartan was used simultaneously with acetaminophen, amlodipine, glibenclamide, simvastatin, hydrochlorothiazide, and ibuprofen. (4) Telmisartan is not metabolized by cytochrome P450 and has no effect on cytochrome P450 convertase in vitro. It only slightly inhibits CYP2C19. Telmisartan does not interact with drugs that inhibit cytochrome P450 convertase.
