Function and Efficacy
This product is a compound preparation of mezlocillin sodium and sulbactam sodium in a ratio of 4:1. Mezlocillin is a broad-spectrum penicillin antibiotic that mainly kills bacteria by interfering with the synthesis of bacterial cell walls; sulbactam has no antibacterial activity against other bacteria except Neisseria and Acinetobacter, but sulbactam has an irreversible inhibitory effect on most important beta-lactamases produced by beta-lactam antibiotic-resistant strains. Sulbactam can prevent the destruction of penicillin and cephalosporin antibiotics by resistant bacteria, and sulbactam has a significant synergistic effect with penicillin and cephalosporin antibiotics. This product has a bactericidal effect on a variety of Gram-positive and Gram-negative bacteria (including aerobic and anaerobic strains), and is stable in vitro to beta-lactamases produced by most bacteria. In vitro tests show that the combination of the two drugs in the compound can enhance the antibacterial effect on a variety of enzyme-producing strains and Staphylococcus aureus and Escherichia coli; compared with mezlocillin alone, this product has different degrees of enhancement in antibacterial effect on Acinetobacter, Alcaligenes faecalis, Serratia marcescens, Aerobacter, Cloacae, Citrobacter, Shigella dysenteriae, Pseudomonas aeruginosa, etc. In addition, mezlocillin also has antibacterial activity against the following microorganisms in vitro: Enterobacter, Proteus mirabilis, Proteus vulgaris, Morganella, Citrobacter, Klebsiella, Haemophilus influenzae, Haemophilus parainfluenzae, Neisseria, Streptococcus pneumoniae, Peptococcus, Peptostreptococcus, Clostridium, Fusobacterium, Polymorpha, etc.

Indication
This product contains the β-lactamase inhibitor - sulbactam, which is suitable for the following moderate to severe infectious diseases caused by enzyme-producing resistant bacteria, including: 1. Respiratory system infections: such as otitis media, sinusitis, tonsillitis, pharyngitis, pneumonia, acute bronchitis and acute exacerbation of chronic bronchitis, bronchiectasis, empyema, lung abscess, etc.; 2. Urinary and reproductive system infections: such as pyelonephritis, cystitis and urethritis, etc.; 3. Abdominal infections: such as biliary tract infections, etc.; 4. Skin and soft tissue infections: such as fossa tissue inflammation, wound infection, furunculosis, purulent dermatitis and vesicular disease; 5. Sexually transmitted diseases, gonorrhea, etc.; 6. Pelvic infections: obstetric infections, postpartum infections, etc.; 7. Severe systemic infections: such as meningitis, bacterial endocarditis, peritonitis, sepsis, sepsis, etc. For fatal systemic bacterial infections, infections caused by unknown or insensitive microorganisms, severe infections and mixed infections, if mezlocillin sodium and sulbactam sodium for injection (4:1) is used, it is recommended to use it in combination with other bactericides for treatment.
Usage and Dosage
Intravenous drip: Dissolve with appropriate amount of injection water or sodium chloride injection before use, then add 0.9% sodium chloride injection or 5% glucose sodium chloride injection or 5%-10% glucose injection 100ml and drip intravenously, each drip time is 30-50 minutes. Adult dose: 3.75g each time (3.0g mezlocillin, 0.75g sulbactam), once every 8 hours or 12 hours, and the course of treatment is 7-14 days.
Adverse Reactions
This product is usually well tolerated, and common adverse reactions are: 1. Gastrointestinal reactions: such as diarrhea, nausea, vomiting, etc. Diarrhea usually disappears during treatment or when the drug is no longer used. When severe and persistent diarrhea occurs, the possibility of potentially fatal antibiotic-induced low-membrane enterocolitis should be considered. At this time, the drug must be discontinued immediately and appropriate treatment must be taken (such as oral vancomycin 4 times; 250 mg/day). Drugs that reduce motility are prohibited. 2. Allergic reactions: Occasionally, allergic reactions occur, usually skin reactions (such as rash, itching). When urticaria occurs (urticaria reaction occurs when allergic to penicillin), the drug must be discontinued. Other penicillin anti-inflammatory drugs should not be continued for treatment. Other rare allergic reactions include eosinophilia, drug-induced fever, and acute interstitial nephritis.
Precautions
It is contraindicated in patients who are allergic to penicillins or sulbactam.
Special Population Medication
Precautions for children: For children aged 1 to 14 years and infants weighing more than 3kg, take 75mg/kg body weight each time, 2-3 times a day. For those weighing less than 3kg, take 75mg/kg body weight each time, 2 times a day. Precautions for pregnancy and lactation: This product can pass through the placental barrier, but no embryotoxicity has been found. Therefore, when using this product, the pros and cons should be weighed. This product can be used during pregnancy under appropriate indications. This product can enter breast milk, so it should be used with caution in pregnant and lactating women. Precautions for the elderly: The adult dose can be referred to, but the dose should be adjusted for patients with poor liver and kidney function.
Drug Interactions
When this product is used simultaneously with high-dose heparin and anticoagulants, coagulation parameters should be monitored. Probenecid can inhibit the renal excretion of this product. When using this product, the patient's non-enzymatic urine glucose reaction, urobilinogen test and urine protein determination may have false positive results. This product should be avoided in combination with drugs with strong acidity or alkalinity (below pH 4.0 or above pH 8.0).
