Function and Efficacy

After entering the body, metronidazole is acted upon by intracellular ion transporters, which form a concentration gradient that promotes the transport of drugs into cells. The drug forms free radicals in the cell, which in turn react with intracellular components and eventually lead to the death of microorganisms. Gram-positive anaerobic bacteria: Clostridium, Eubacterium, Peptococcus nigra, Peptostreptococcus. Gram-negative anaerobic bacteria: Bacteroides fragilis (type species, Bacteroides divaricata, Bacteroides ovatus, Bacteroides thetaiotaomicron and Bacteroides vulgaris), Prevotella (Prevotella biloba, Prevotella buccalis, Prevotella saccharolyticus), Porphyromonas, Fusobacterium. Parasitic protozoa: Entamoeba histolytica, Trichomonas vaginalis.

It also has a killing effect on parasitic protozoa such as Entamoeba histolytica and Trichomonas vaginalis.

Indication

Treat bacterial vaginosis in women.

Usage and Dosage

Oral administration: 750 mg (1 tablet) once a day for seven consecutive days, with seven days as a course of treatment. Metronidazole sustained-release tablets should be swallowed whole on an empty stomach at least 1 hour before or 2 hours after a meal. Taking on an empty stomach can maintain the best sustained-release properties of this product.

Adverse Reactions

Most adverse events were mild to moderate, including headache, nausea, metallic taste, and vulvar and vaginal candidiasis.

Precautions

People who are allergic to metronidazole or other nitroimidazole derivatives; women who are pregnant within three months.

Special Population Medication

Precautions for children: The safety of metronidazole sustained-release formulations in pediatric patients has not been determined. Precautions for pregnancy and lactation: Pregnant mice were given metronidazole (60 mg/m2 per body surface area, approximately 10% of the clinical dose) once a day by oral gavage, which did not produce fetal toxicity. However, intrauterine death has occurred in animal studies of single-dose intraperitoneal administration, and it is not certain whether it is related to the medication. Due to the lack of in-depth comparative studies on pregnant women, and animal studies do not always predict human responses, metronidazole is a rodent carcinogen, and metronidazole can quickly cross the placental barrier into the fetal circulation, so it should be avoided within three months of pregnancy. Since metronidazole can be secreted in breast milk, the concentration is similar to that in plasma, and since the incidence of tumors in mice and rats increased after metronidazole was used, it is necessary to weigh the pros and cons based on the importance of the drug and the mother to decide whether to terminate lactation or stop the medication. Precautions for the elderly: It is necessary to monitor blood drug levels in elderly patients to adjust the dosage of metronidazole.
