Ganciclovir eye gel

Function and Efficacy

Ganciclovir is an analog of 2'-deoxyguanine nucleotides that inhibits the replication of herpes viruses. Its mechanism of action is: ganciclovir is first phosphorylated into a monophosphate by a protein kinase homolog encoded by CMV (UL97 gene), and then further phosphorylated into a diphosphate and a triphosphate by cellular kinases. In CMV-infected cells, the level of triphosphate is 100 times higher than that in non-infected cells, suggesting that this product can be preferentially phosphorylated in infected cells. Once ganciclovir forms a triphosphate, it can persist for several days in CMV-infected cells. The triphosphate of ganciclovir is believed to inhibit viral DNA synthesis in the following ways: 1) competitively inhibiting viral DNA polymerase; 2) co-entering viral DNA, thereby leading to the termination of viral DNA elongation.

Appearance

This product is a water-soluble, colorless, transparent gel.

Indication

Herpes simplex keratitis.

Usage and Dosage

Usage: For external use, apply into the conjunctival sac. Dosage: About 8mm at a time, 4 times a day, 3 weeks of treatment.

Adverse Reactions

During treatment, there may be a brief itching, burning sensation, tingling sensation and slight blurred vision, but they disappear quickly and do not affect the treatment. Occasionally, a decrease in white blood cells is seen.

Precautions

It is contraindicated for patients who are allergic to ganciclovir, patients with severe neutropenia (less than 0.5 times; 109/L) or severe thrombocytopenia (less than 25 times; 109/L).

Special Population Medication

Precautions for children: There is a lack of data on the use of ganciclovir in children, so it is recommended that children use it with caution. Consult a doctor before use and use it when the potential benefits outweigh the risks. Precautions for pregnancy and lactation: Animal experiments have shown that oral and intravenous administration of ganciclovir is teratogenic and reproductively toxic, so pregnant women should weigh the pros and cons before deciding whether to use the drug. Use with caution in lactating women, and consult a doctor before using this product. Precautions for the elderly: There is a lack of literature on the use of this drug in elderly patients.

Drug Interactions

Because no relevant studies have been conducted, the interaction between this product and other ophthalmic drugs is still unclear. When ganciclovir is administered orally, the drug interactions are: Didanosine: Taking didanosine 2 hours before or at the same time as this product can increase the steady-state AUC0-12 of didanosine by 111plusmn; 114%. Taking didanosine 2 hours before taking this product orally can reduce the steady-state AUC of ganciclovir by 21plusmn; 17%, but the AUC of ganciclovir is not affected when the two drugs are used simultaneously, and the renal clearance of both drugs is not significantly changed. Zidovudine: When this product is taken orally at a dose of 1000 mg every 8 hours and zidovudine at a dose of 100 mg every 4 hours, the average steady-state AUC0-8- of ganciclovir decreases by 17plusmn; 25%. The steady-state AUC0-4 of zidovudine increases by 19plusmn; 27%. Because both zidovudine and ganciclovir may cause neutropenia and anemia, some patients may not be able to tolerate the full dose combination of the two drugs. Probenecid: When 1000 mg of this product is taken orally every 8 hours and 500 mg of probenecid is taken orally every 6 hours, the mean steady-state AUC0-8- of ganciclovir increases by 53 plus mn; 91%, and the renal clearance decreases by 22 plus mn; 20%. This interaction is related to competition for tubular secretion. Imipenem-cilastatin: There have been reports of patients receiving this product and imipenem-cilastatin simultaneously experiencing seizures without significant characteristics. Therefore, these drugs should not be used simultaneously unless the potential benefits outweigh the risks. Other drugs: Drugs that inhibit the replication of rapidly dividing cell populations such as bone marrow, spermatogonia, skin germinal layer, and gastrointestinal mucosal cells can increase toxicity when used in combination with ganciclovir. Therefore, such drugs as dapsone, pentamidine, 5-fluorocytosine, vincristine, vinblastine, doxorubicin, amphotericin B, trimethoprim/sulfamethoxazole combination, or other nucleoside analogs should only be used concomitantly with this product when the potential benefit outweighs the risk.

Storage

Store in a sealed container at a temperature above 10°C. The shelf life after opening the tube should not exceed 4 weeks.
