Function and Efficacy
Quinine is a quinoline derivative that can bind to the DNA of malarial parasites to form a complex that inhibits DNA replication and RNA transcription, thereby inhibiting the protein synthesis of the protozoa. Its effect is weaker than that of chloroquine. In addition, quinine can reduce the oxygen consumption of malarial parasites, resist the phosphorylase in malarial parasites and interfere with their sugar metabolism. Quinine also causes hemochromatin aggregation, but it develops slowly, rarely forms large clumps, and is often accompanied by cell death. Electron microscopic observation shows that the nucleus and outer membrane of the protozoa are swollen, and there are small vacuoles. The blood cell particles aggregate in the small vacuoles, which is different from the pigment aggregation of chloroquine. In the blood, a certain concentration of quinine can cause premature rupture of parasitized red blood cells, thereby preventing the maturation of schizonts. This product is ineffective in the early stage of malaria, and a long course of treatment can cure malignant malaria, but it has no direct effect on the gametocytes of malignant malaria, so it cannot interrupt transmission.

Indication
It is used to treat falciparum malaria caused by chloroquine-resistant and multi-drug-resistant strains. It can also be used to treat vivax malaria.
Usage and Dosage
1 The adult dosage for the treatment of malignant malaria caused by chloroquine-resistant strains is 1.8 g per day, taken in divided doses, for a course of 14 days. 2 The common dosage for children for the treatment of malignant malaria caused by chloroquine-resistant strains is 0.1-0.2 g per day for children under 1 year old, taken in 2-3 divided doses, 0.2-0.3 g for children aged 1-3 years old, 0.3-0.5 g for children aged 4-6 years old, and 0.5-1 g for children aged 7-11 years old, for a course of 10 days.
Adverse Reactions
1. Quinine daily dosage of more than 1g or long-term use often causes cinchona reaction, which is roughly similar to salicylic acid reaction, with symptoms such as tinnitus, headache, nausea, vomiting, vision and hearing loss, etc. In severe cases, temporary deafness occurs, which can often recover after stopping the drug; when the dose is greater than 4g within 224 hours, it can directly damage nerve tissue and constrict retinal blood vessels, resulting in narrowing of visual field, diplopia, amblyopia, etc.; 3. When poisoned by large doses, in addition to the aggravation of the above reactions, it can also inhibit the myocardium, prolong the refractory period, slow conduction, weaken myocardial contractility, dilate peripheral blood vessels, and sometimes cause a sudden drop in blood pressure, slow and shallow breathing, fever, irritability, delirium, etc., and most people die from respiratory paralysis; 4. The lethal dose of quinine is about 8g; 5. A few patients are highly sensitive to quinine, and a small amount can cause severe cinchona reaction; 6. A small number of patients with malignant malaria may suffer from acute hemolysis (blackwater fever) and die after using a small amount of quinine; 7. Quinine can also cause rash, itching, asthma, etc.
Precautions
It is forbidden for pregnant women to use.
Drug Interactions
1. Antacids and aluminum-containing preparations can delay or reduce the absorption of quinine; 2. When anticoagulants are used in combination with quinine, the anticoagulant effect can be enhanced; 3. Muscle relaxants such as succinylcholine and tubocurarine can react with quinine and may cause respiratory depression; 4. When quinidine is used in combination with quinine, the cinchona reaction can be increased; 5. Urine alkalinizing agents such as sodium bicarbonate can increase the reabsorption of quinine by the renal tubules, leading to an increase in quinine blood concentration and toxicity; 6. Combination with vitamin K can increase the absorption of quinine; 7. Combination with buclizine, cyclizine, meclizine, phenothiazines, thioxanthenes, trimethofenzamide, and aminoglycoside antibiotics can cause tinnitus and dizziness; 8. When used in combination with nifedipine (nifedipine), the concentration of free quinine increases.
