Function and Efficacy

This product is an aminoglycoside antibiotic. It has good antibacterial effects on various Gram-negative and Gram-positive bacteria, and has good antibacterial effects on various Enterobacteriaceae such as Escherichia coli, Klebsiella, Proteus, Salmonella, Shigella, Enterobacter, Serratia and Pseudomonas aeruginosa. Neisseria and Haemophilus influenzae are moderately sensitive to this product. It also has a certain effect on Brucella, Yersinia pestis, Acinetobacter and Campylobacter fetus. It has a good antibacterial effect on about 80% of methicillin-sensitive strains of Staphylococcus (including Staphylococcus aureus and coagulase-negative Staphylococcus), but methicillin-resistant strains are mostly resistant to this product. It has a poor effect on Streptococcus and Streptococcus pneumoniae, and Enterococcus is mostly resistant to this product. When this product is used in combination with β-lactams, most of them can obtain synergistic antibacterial effects. The mechanism of action of this product is to bind to the 30S subunit of the bacterial ribosome and inhibit the synthesis of bacterial proteins. In recent years, the number of Gram-negative bacteria resistant to gentamicin has increased significantly.

Usage and Dosage

Before use, add injection water to dissolve. Intravenous drip or intramuscular injection 40-80 mg (40,000-80,000 units) once, 2-4 times a day. Children are injected 3-5 mg (3,000-5,000 units) per kg of body weight per day, divided into 2-4 injections.

Adverse Reactions

1. Ototoxic reactions such as hearing loss, tinnitus or fullness in the ears may occur during medication. When vestibular function is affected, unsteady gait and dizziness may occur. Nephrotoxic reactions such as hematuria, significantly reduced urination frequency or urine volume, loss of appetite, and extreme thirst may also occur. Those with a lower incidence include dyspnea, drowsiness, weakness, etc. caused by neuromuscular blockade or nephrotoxicity. Occasionally, there may be rash, nausea, vomiting, decreased liver function, leukopenia, granulocytopenia, anemia, hypotension, etc. 2. A small number of patients may experience ototoxic symptoms such as hearing loss, tinnitus or fullness in the ears after discontinuation of the drug, which should be paid attention to. 3. Systemic administration combined with intrathecal injection may cause leg cramps, rash, fever, and systemic spasms.

Precautions

1. It is contraindicated for those who are allergic to this product or other aminoglycosides; 2. It should be used with caution in premature infants and newborns; 3. It should be used with caution in pregnant women.

Drug Interactions

1. When used in combination with other aminoglycosides or when used topically or systemically successively, it may increase the possibility of producing ototoxicity, nephrotoxicity and neuromuscular blockade. 2. When used in combination with neuromuscular blockers, it may aggravate neuromuscular blockade, leading to symptoms such as muscle weakness and respiratory depression. 3. When used in combination with capreomycin, cisplatin, ethacrynic acid, furosemide or vancomycin (or norvancomycin), or when used topically or systemically successively, it may increase ototoxicity and nephrotoxicity. 4. When used topically or systemically with cephalothin or cefazolin, it may increase nephrotoxicity. 5. When used in combination with polymyxin injections or when used topically or systemically successively, it may increase nephrotoxicity and neuromuscular blockade. 6. Other nephrotoxic and ototoxic drugs should not be used in combination with this product or when used topically successively, so as not to aggravate nephrotoxicity or ototoxicity. 7. When aminoglycosides and β-lactams (cephalosporins and penicillins) are mixed, they may lead to mutual inactivation. When this product is used in combination with the above antibiotics, it must be dripped in separate bottles. This product should not be dripped in the same bottle with other drugs.
