Function and Efficacy

Sulfadiazine is a sulfonamide antibiotic that inhibits bacterial growth and reproduction. It has a strong inhibitory effect on meningococci, pneumonia, streptococci, and hemolytic streptococci, but has poor efficacy on staphylococcal infections. Bacteria can develop resistance to this product. The solubility of its metabolite acetylated product in the body is low, and it is easy to precipitate crystals in the urinary tract. When used in combination with sodium bicarbonate, it makes the urine alkaline, reducing the precipitation of crystals. Sulfadiazine is a sulfonamide antibiotic used for intestinal infections. Its antibacterial mechanism is that it is similar to para-aminobenzoic acid (PABA) in structure, and can compete with PABA on dihydrofolate synthase in bacteria, thereby preventing PABA from being integrated into folic acid required by bacteria as a raw material, reducing the amount of metabolically active tetrahydrofolate, which is an essential substance for bacteria to synthesize purine, thymidine and deoxyribonucleic acid (DNA), thereby inhibiting the growth and reproduction of bacteria. Pharmacokinetics: Sulfadiazine is easily absorbed from the gastrointestinal tract after oral administration, and can absorb more than 70% of the administered dose, but the absorption is slow. The blood concentration reaches the peak 3 to 6 hours after administration, and the protein binding rate is 38% to 48%. This product is used in combination with other drugs. After absorption, it can be widely distributed in the body tissues and body fluids, and penetrate the blood-cerebrospinal fluid barrier to the cerebrospinal fluid to achieve a therapeutic effect. It can also cross the blood-placental barrier and enter the fetal blood circulation. The half-life (t1/2) of this product is about 10 hours, and can reach 34 hours in patients with renal failure. Within 48 to 72 hours after administration, 60% to 85% of the administered dose is excreted in the urine as the original drug. After oral administration of this product, most of the sulfadiazine is excreted in the feces in its original form.

Indication

Used for upper respiratory tract infections, enteritis, otitis media, conjunctivitis, furuncle, and abscesses.

Usage and Dosage

Oral administration. Under 1 year old, half a pack; 1-2 years old, 1 pack; 3-5 years old, 1.5 packs; 6-8 years old, 2 packs; 9-12 years old, 2.5 packs. 4 times a day, double the dose for the first time.

Adverse Reactions

1. Allergic reactions are relatively common. They may manifest as drug rashes. In severe cases, exudative erythema multiforme, exfoliative dermatitis, and epidermolysis bullosa atrophic dermatitis may occur. There are also serum sickness-like reactions such as photosensitivity reactions, drug fever, joint and muscle pain, and fever. 2. Granulocytopenia or deficiency, thrombocytopenia, and aplastic anemia. They manifest as sore throat, fever, pallor, and bleeding tendency. 3. Hemolytic anemia and hemoglobinuria. They are more likely to occur in patients who lack 6-phosphate glucose dehydrogenase after using sulfonamides, and are more common in newborns and children than in adults. 4. Hyperbilirubinemia and neonatal kernicterus. Because sulfonamides compete with bilirubin for protein binding sites, free bilirubin can increase. Newborns have imperfect liver function and poor bilirubin processing, so they are more likely to develop hyperbilirubinemia and neonatal kernicterus.

Precautions

1. Patients who are allergic to sulfonamides are contraindicated. 2. Patients with severe liver and kidney dysfunction are contraindicated. 3. Patients with megaloblastic anemia are contraindicated.

Special Population Medication

Precautions for children: This product can compete with bilirubin for binding sites on plasma proteins, and the acetyltransferase system of newborns is not fully developed, and the free blood concentration of sulfonamide increases, which increases the risk of kernicterus. It should be prohibited for newborns and infants under 2 months old. Precautions for pregnancy and lactation: Not yet clear. Precautions for the elderly: Not yet clear.

Drug Interactions

1. When used simultaneously with urine alkalinizing drugs, the solubility of this product in alkaline urine can be increased, which increases excretion. 2. It cannot be used together with para-aminobenzoic acid. Para-aminobenzoic acid can replace this product and be taken up by bacteria, thus antagonizing the antibacterial effect of this product. The two should not be used together. 3. When used together with anticoagulants, oral hypoglycemic drugs, methotrexate, phenytoin sodium and thiopental sodium, the latter can replace the protein binding sites of these drugs, or inhibit their metabolism, resulting in prolonged drug action or toxicity. Therefore, when used together with these drugs or after using this product, its dosage needs to be adjusted. 4. When bone marrow suppressive drugs are used simultaneously with this product, the adverse reactions of such drugs on the hematopoietic system may be enhanced.
