Appearance

This product is white or off-white tablets.

Indication

1. Hypertension; 2. Heart failure.

Usage and Dosage

Depends on the condition or individual differences. This product should be taken under the guidance or supervision of a physician. The dosage must follow the principle of individualization and be adjusted according to the efficacy. 1. Common dosage for adults: (1) For hypertension, take 12.5 mg (half a tablet) orally once, 2-3 times a day, and increase to 50 mg (2 tablets) 2-3 times a day within 1-2 weeks as needed. If the efficacy is still unsatisfactory, other antihypertensive drugs can be added. (2) For heart failure, start with 12.5 mg (half a tablet) orally once, 2-3 times a day, and gradually increase to 50 mg (2 tablets) 2-3 times a day if necessary. If further increase in dosage is required, it is advisable to observe the efficacy for 2 weeks before considering: For patients who have recently taken a large amount of diuretics, are in hyponatremia/hypovolemia, and have normal or low blood pressure, the initial dose should be 6.25 mg (1/4 tablet), 3 times a day. Later, it can be gradually increased to the common dosage through testing. 2. Common dosage for children: For lowering blood pressure and treating heart failure, start with 0.3 mg/kg body weight, 3 times a day. If necessary, increase the dosage by 0.3 mg/kg every 8-24 hours to obtain the lowest effective dose.

Adverse Reactions

1. The more common ones are: (1) Rash, which may be accompanied by itching and fever, and often occurs within 4 weeks of treatment. It is maculopapular or urticaria, and disappears after reducing the dose, stopping the drug or taking antihistamines. 7% to 10% are accompanied by eosinophilia or positive antinuclear antibodies. (2) Palpitations, tachycardia, chest pain. (3) Cough. (4) Taste loss. 2. Less common ones are: (1) Proteinuria, which often occurs within 8 months of the start of treatment. Among them, 1/4 develop nephrotic syndrome, but proteinuria gradually decreases within 6 months and the course of treatment is not affected. (2) Dizziness, headache, and syncope. It is caused by hypotension, especially when there is a lack of sodium or blood volume. (3) Angioedema, which occurs on the face and limbs, and can also cause angioedema of the tongue, glottis or larynx, so be vigilant. (4) Rapid and irregular heart rate. (5) Facial flushing or pallor. 3. Rare symptoms include: leukopenia and granulocytopenia, fever, chills. Leukopenia is dose-related and occurs 3-12 weeks after the start of treatment, with the most significant symptoms occurring 10-30 days after the start of treatment. It lasts for 2 weeks after the drug is stopped. Patients with renal failure should be more vigilant, as taking allopurinol together may increase this risk.

Precautions

Patients who are allergic to this product or other angiotensin-converting enzyme inhibitors are contraindicated.

Special Population Medication

Precautions for children: There have been reports that this product can cause excessive and persistent decreases in blood pressure in infants, accompanied by oliguria and convulsions, so the use of this product is limited to those who have not responded to other antihypertensive treatments. Precautions for pregnancy and lactation: 1. This product can pass through the placenta. 2. This product can be excreted into breast milk, and its concentration is about 1% of the maternal blood drug concentration, so lactating women must weigh the pros and cons when using it. 3. Pregnant women who absorb ACEⅠ can affect fetal development and even cause fetal death, so pregnant women are prohibited from using it. Precautions for the elderly: The elderly are more sensitive to antihypertensive effects, so the dosage of this product should be reduced as appropriate.

Drug Interactions

1. Used with diuretics to enhance the antihypertensive effect. However, it should be avoided to cause severe hypotension. Therefore, those who originally used diuretics should stop taking them or reduce the dosage. This product should be used at a small dose at the beginning and the dosage should be adjusted gradually. 2. Used with other vasodilators may cause hypotension. If used together, it should be started at a small dose. 3. Used with potassium-retaining drugs such as spironolactone, triamterene, and amiloride may cause hyperkalemia. 4. Used with endogenous prostaglandin synthesis inhibitors such as indomethacin. This will weaken the antihypertensive effect of this product. 5. Used with other antihypertensive drugs, the antihypertensive effect is enhanced. Used with drugs that affect sympathetic nerve activity (ganglionic blockers or adrenergic nerve blockers) and beta-blockers will cause the antihypertensive effect to be enhanced, so be vigilant. 6. Combined with lithium agents, serum lithium levels may increase and toxicity may occur.
