Function and Efficacy
Antiviral drug. It has an inhibitory effect on herpes simplex virus, varicella zoster virus, cytomegalovirus, etc. in vitro. After entering the cells infected by herpes virus, this product competes with deoxynucleoside for viral thymidine kinase or cell kinase, and the drug is phosphorylated into activated acyclovir triphosphate, and then inhibits viral replication in two ways: ① Interfering with viral DNA polymerase to inhibit viral replication; ② Under the action of DNA polymerase, it binds to the growing DNA chain, causing the extension of the DNA chain to be interrupted. This product has a special affinity for viruses, but has low toxicity to mammalian host cells. There are reports of carcinogenicity in in vitro cell transformation assays, but no evidence of carcinogenicity has been found in animal experiments. Some animal experiments show that high concentrations of drugs can cause mutations, but there is no evidence of chromosomal changes. The carcinogenic and mutagenic effects of this product are still unclear. High-dose injection can cause testicular atrophy and reduced sperm count in animals. The drug can pass through the placenta, and animal experiments have confirmed that it has no effect on the embryo.

Appearance
This product is white loose mass or powder.
Indication
1. Herpes simplex virus infection: used for the treatment of primary and recurrent mucocutaneous infections in immunocompromised individuals and the prevention of recurrent cases; also used for the treatment of herpes simplex encephalitis. 2. Herpes zoster: used for the treatment of severe herpes zoster in immunocompromised individuals or diffuse herpes zoster in immunocompetent individuals. 3. Treatment of varicella in immunocompromised individuals. 4. Treatment of acute retinal necrosis.
Usage and Dosage
Intravenous drip, each drip time is more than 1 hour. Common dosage for adults 1. For the initial treatment of severe genital herpes, 5 mg/kg (calculated as acyclovir, the same below) once according to body weight, 3 times a day, drip once every 8 hours, for a total of 5 days. 2. For immunocompromised patients with skin and mucous membrane herpes simplex or severe herpes zoster, 5-10 mg/kg once according to body weight, 3 times a day, drip once every 8 hours, for a total of 7-10 days. 3. For herpes simplex encephalitis, 10 mg/kg once according to body weight, 3 times a day, drip once every 8 hours, for a total of 10 days. 4. For acute retinal necrosis, 5-10 mg/kg once, 3 times a day, drip once every 8 hours, for a total of 7-10 days. Later, take 0.8 g orally once, 5 times a day, for 6-14 weeks. The maximum daily dose for adults is 30 mg/kg according to body weight, or 1.5 g/m2 according to body surface area. Common dosage for children 1. For the initial treatment of severe genital herpes, infants and children under 12 years old should take 250mg/m2 of body surface area once (calculated as acyclovir, the same below), 3 times a day, drip once every 8 hours, for a total of 5 days. 2. For herpes simplex of skin and mucous membranes in immunocompromised patients, infants and children under 12 years old should take 250mg/m2 of body surface area once, 3 times a day, drip once every 8 hours, for a total of 7 days, and for those over 12 years old, use the adult dosage. 3. For herpes simplex encephalitis, take 10mg/kg of body weight once, 3 times a day, drip once every 8 hours, for a total of 10 days. 4. For immunocompromised patients with varicella, take 10mg/kg of body weight once or 500mg/m2 of body surface area once, 3 times a day, drip once every 8 hours, for a total of 10 days. The maximum dose for children is 500mg/m2 of body surface area every 8 hours. Preparation of the drug solution: Take 0.5g of this product and add it to 10ml of water for injection to make the concentration of 50g/L. After fully shaking into a solution, dilute it to at least 100ml with sodium chloride injection or 5% glucose injection. Make sure that the final drug concentration does not exceed 7g/L, otherwise it is easy to cause phlebitis.
Adverse Reactions
1 Common adverse reactions: Inflammation or phlebitis at the injection site, itching or urticaria, rash, fever, mild headache, nausea, vomiting, diarrhea, proteinuria, increased blood urea nitrogen and serum creatinine, abnormal liver function such as mild increase in serum aminotransferase, alkaline phosphatase, lactate dehydrogenase, and total bilirubin. 2 Rare adverse reactions include: acute renal insufficiency, decreased white blood cells and red blood cells, decreased hemoglobin, increased cholesterol and triglycerides, hematuria, hypotension, hyperhidrosis, palpitations, dyspnea, chest tightness, etc. 3 Rare adverse reactions include: coma, confusion, hallucinations, epilepsy, lower limb convulsions, numbness of the tongue and hands and feet, tremor, general fatigue and other central nervous system symptoms.
Precautions
It is contraindicated for those who are allergic to this product.
Drug Interactions
1. When used in combination with interferon or methotrexate (intrathecal), it may cause mental abnormalities and should be used with caution. 2. When used in combination with nephrotoxic drugs, it can aggravate nephrotoxicity, especially in patients with renal insufficiency. 3. When used in combination with zidovudine, it can cause nephrotoxicity, manifested as deep lethargy and fatigue. 4. It competitively inhibits the secretion of organic acids with probenecid. When used in combination with probenecid, it can slow down the excretion of this product, prolong its half-life, and cause drug accumulation in the body.
