    Ivermectin is a medication that is effective against many types of parasites. It is used to treat head lice, scabies, river blindness, strongyloidiasis, and lymphatic filariasis, among others. Ivermectin and other avermectins (insecticides most frequently used in home-use ant baits) are macrocyclic lactones derived from the bacterium Streptomyces avermitilis.     Introduction

    Ivermectin is a broad-spectrum antiparasitic agent widely used in both human and veterinary medicine. It is notably effective against a variety of internal and external parasites, including nematodes (roundworms), arthropods (such as lice and mites), and some ectoparasites. It is best known for its use in treating human diseases caused by parasitic worms, including onchocerciasis (river blindness) and strongyloidiasis, as well as its role in mass drug administration programs for neglected tropical diseases.

    Origin: Derived from avermectins, a group of compounds isolated from Streptomyces avermitilis

    Route of administration: Oral, topical (cream/lotion), or subcutaneous (veterinary use)

    Mechanism of Action

    Ivermectin works by binding to and activating glutamate-gated chloride ion channels found in the nerve and muscle cells of invertebrates. This increases the permeability of the cell membrane to chloride ions, leading to hyperpolarization of the nerve or muscle cell, paralysis, and eventually death of the parasite.

    The drug is selective for parasites because these glutamate-gated channels are absent in mammals. Additionally, ivermectin does not significantly cross the blood-brain barrier in humans due to the presence of P-glycoprotein transporters, minimizing effects on the central nervous system.

    Indications and Uses in Humans

    Ivermectin is FDA-approved or globally used for treating the following conditions:

    Onchocerciasis (river blindness): Caused by the filarial worm Onchocerca volvulus

    Strongyloidiasis: Intestinal infection caused by Strongyloides stercoralis

    Lymphatic filariasis: Often used in combination with albendazole

    Scabies: Particularly in resistant or crusted (Norwegian) scabies

    Pediculosis (head lice): Topical ivermectin lotion (Sklice®)

    Cutaneous larva migrans: Migrating skin infection caused by hookworm larvae

    Other off-label uses: Rosacea (topical ivermectin), myiasis, and preventive treatment in endemic settings

    Ivermectin is also a central component of mass drug administration strategies for controlling parasitic diseases in public health programs (e.g., WHO’s program to eliminate onchocerciasis).

    Note: While ivermectin gained global attention during the COVID-19 pandemic, its use for COVID-19 remains controversial and is not recommended by major health authorities, including the WHO and FDA, outside clinical trial settings.

    Dosage and Administration

    Dosing depends on the condition being treated and the patient’s body weight.

    For onchocerciasis: 150 µg/kg orally as a single dose, repeated every 6 to 12 months

    For strongyloidiasis: 200 µg/kg orally once daily for 1–2 days

    For scabies: 200 µg/kg orally as a single dose, repeated after 7–14 days if needed

    For pediculosis: Topical application of 0.5% ivermectin lotion to hair and scalp, left on and rinsed based on product instructions

    For rosacea: 1% ivermectin cream applied once daily to affected facial areas

    Oral ivermectin is typically taken on an empty stomach with water. Dosing must be carefully calculated based on the patient’s weight.

    Pharmacokinetics

    Absorption: Orally administered ivermectin is well absorbed, especially when taken with food (although traditionally taken on an empty stomach for consistent absorption).

    Peak plasma concentration: Occurs within 4–5 hours after oral administration.

    Distribution: Highly lipophilic and widely distributed in body tissues, especially fat.

    Metabolism: Primarily metabolized in the liver via cytochrome P450 (mainly CYP3A4).

    Elimination: Mainly via feces (biliary route); half-life is approximately 18 hours.

    Common Side Effects

    When used appropriately, ivermectin is generally well tolerated. Common side effects include:

    Headache

    Dizziness or lightheadedness

    Gastrointestinal symptoms (e.g., nausea, abdominal pain, diarrhea)

    Skin rash or itching (particularly in patients with onchocerciasis, due to the immune reaction to dying microfilariae)

    Serious or Rare Adverse Effects

    Mazzotti reaction: An intense systemic allergic/inflammatory response in onchocerciasis patients, caused by the rapid killing of microfilariae. Symptoms include fever, hypotension, joint pain, lymph node swelling, and rash. It is not caused by the drug itself, but rather by the host immune response.

    Hepatotoxicity: Liver enzyme elevations are rare but have been reported.

    Neurotoxicity: Rare cases of encephalopathy have occurred, particularly in patients co-infected with Loa loa (a different filarial parasite).

    Severe skin reactions: Stevens-Johnson syndrome or toxic epidermal necrolysis (very rare)

    Contraindications and Precautions

    Ivermectin should not be used in individuals with:

    Known hypersensitivity to ivermectin or components of its formulation

    Children weighing less than 15 kg (for oral forms, unless specifically approved)

    Severe liver disease (relative contraindication—requires caution and monitoring)

    Meningitis or Loa loa infection (due to risk of severe neurologic complications)

    Because it is primarily metabolized in the liver, patients with hepatic impairment should be monitored carefully.

    Drug Interactions

    Ivermectin is metabolized by CYP3A4 and is a substrate for P-glycoprotein, so drug interactions may occur with:

    CYP3A4 inducers (e.g., rifampin, phenytoin)
