Rosuvastatin calcium is a synthetic lipid-lowering agent belonging to the class of HMG-CoA reductase inhibitors (statins), used primarily to treat hyperlipidemia and reduce cardiovascular risk.

Mechanism of Action

Rosuvastatin selectively and competitively inhibits HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis that converts HMG-CoA to mevalonate. This inhibition results in:
Decreased cholesterol synthesis in the liver
Upregulation of hepatic LDL receptors, increasing clearance of LDL cholesterol (LDL-C) from the bloodstream
Reduced synthesis of very-low-density lipoprotein (VLDL), lowering triglyceride levels
Increased high-density lipoprotein cholesterol (HDL-C) levels
Additional pleiotropic effects including anti-inflammatory actions, improved endothelial function via increased nitric oxide bioavailability, and reduced oxidative stress

Indications and Uses

Hypertriglyceridemia: Adjunct to diet for adult patients to lower elevated triglycerides
Primary dysbetalipoproteinemia (Type III hyperlipoproteinemia): Adjunct to diet
Homozygous familial hypercholesterolemia: Adjunct or monotherapy to reduce LDL-C, total cholesterol, and apolipoprotein B
General hypercholesterolemia and mixed dyslipidemia: To reduce cardiovascular morbidity and mortality by lowering LDL-C and other atherogenic lipoproteins

Safety and Side Effects

At the 40 mg dose, higher rates of myalgia (12.7%), arthralgia (10.1%), headache (6.4%), dizziness (4.0%), and elevated liver enzymes (ALT >3x ULN in 2.2%) were observed compared to placebo.
Rare but serious adverse effects include:
Myopathy and rhabdomyolysis, especially at higher doses or in patients with risk factors (e.g., age ≥65, renal impairment, hypothyroidism, concomitant fibrates or certain antivirals)
Immune-mediated necrotizing myopathy (IMNM), a rare autoimmune condition with persistent muscle weakness and elevated creatine kinase despite drug discontinuation
Elevated liver enzymes and possible hepatotoxicity
Hypersensitivity reactions including rash, urticaria, and angioedema
Pancreatitis (rare)

Precautions

Monitor for muscle symptoms; discontinue therapy if myopathy or markedly elevated creatine kinase occurs
Use caution in patients with renal impairment, hypothyroidism, or those taking interacting drugs (e.g., fibrates, cyclosporine, protease inhibitors, colchicine)
Regular monitoring of liver function tests recommended
Not studied in Fredrickson Type I and V dyslipidemias
