Risperidone is used to treat schizophrenia, bipolar disorder, or irritability associated with autistic disorder. 

Mechanism of action

Risperidone is a selective monoamine antagonist that has a high affinity for 5-hydroxytryptaminergic 5-HT2 receptors and dopaminergic D2 receptors, and can also bind to alpha 1-adrenergic receptors. It has a lower affinity for H1 histamine receptors and alpha 2-adrenergic receptors, but cannot bind to cholinergic receptors. Risperidone is a potent D2 antagonist, which is why it inhibits positive symptoms of schizophrenia. However, it causes less motor function inhibition and tonic syncope than classical antipsychotic drugs. Balancing the antagonistic effects of serotonin and dopamine on the central system can alleviate the occurrence of extrapyramidal reactions and extend its therapeutic effects to negative and emotional symptoms of schizophrenia.

Side Effects

Common side effects include the occurrence of extrapyramidal syndrome, drowsiness, constipation, eye reflux, and weight gain. Serious side effects include permanent behavioral disorders, late-onset motor disorders such as antipsychotic malignant syndrome, increased risk of suicide, and high blood sugar. Middle aged and elderly patients with mental illness who develop dementia may have an increased risk of death due to this condition.
