Secondary open-angle glaucoma caused by the spread of pigment in the anterior segment is called pigmentary glaucoma. In 1949, Sugar made a detailed report on the pigment spread syndrome and proposed the name of pigmentary glaucoma. This type of glaucoma accounts for 1.0% to 1.5% of glaucoma in Western countries, but it is less common in glaucoma in my country. It is characterized by the loss of pigment on the posterior surface of the iris in the periphery of both eyes, accompanied by pigment deposition in intraocular tissues such as the back of the cornea, trabecular meshwork, iris and lens. The iris is examined by transillumination to show a spoke-shaped translucent area. The pigment is released from the iris pigment epithelium and circulated through the aqueous humor and deposited on the lens suspension ligament, lens, posterior wall of the cornea, anterior surface of the iris, trabecular meshwork and peripheral retina. After a long time, the pigment accumulates in the aqueous humor drainage duct.
