Adefovir dipivoxil

Adefovir is an acyclic phosphorylated nucleoside analog of adenosine monophosphate. It is phosphorylated to an active metabolite, adefovir diphosphate, under the action of cellular kinases. It inhibits HBV DNA polymerase (reverse transcriptase) by competing with the natural substrate deoxyadenosine triphosphate and causing DNA chain elongation termination after integration into viral DNA. It is still effective against HBV variants containing lamivudine resistance-related mutations.

TOLBUTAMIDE

1. Stimulate the pancreatic beta cells to secrete insulin, the prerequisite is that the pancreatic beta cells still have a certain ability to synthesize and secrete insulin; 2. By increasing the level of portal vein insulin or directly acting on the liver, inhibiting liver glycogenolysis and gluconeogenesis, the liver produces and outputs less glucose; 3. It may also increase the sensitivity of extra-pancreatic tissues to insulin and the utilization of sugar (probably mainly through post-receptor effects). Therefore, the overall effect is to lower fasting blood sugar and postprandial blood sugar.

It is one of the components of the connective tissue matrix in organisms. Its mechanism of action may be related to inhibiting phosphodiesterase activity, increasing cyclic adenosine monophosphate content and activating lipolytic enzymes. It can promote peripheral circulation and regulate cell growth.

It has special effects on Staphylococci (such as Staphylococcus aureus, Staphylococcus epidermidis and penicillin-resistant strains), Streptococci (such as Group A hemolytic streptococci, non-hemolytic streptococci and Streptococcus pneumoniae), Pseudomonas aeruginosa, Escherichia coli, Klebsiella pneumoniae, Escherichia coli, Proteus mirabilis (indole-negative) and indole-positive Proteus, Haemophilus influenzae and Haemophilus conjunctivitis, Moraxella conjunctivitis, Acinetobacter calcoaceticus (Herellea vaginacola), Neisseria, etc. Bacteria resistant to gentamicin are still sensitive to tobramycin. Long-term use may cause bacterial resistance.
