Function and Efficacy
Cefixime is a third-generation oral cephalosporin that kills bacteria by inhibiting bacterial cell wall synthesis. It is stable to most β-lactamases, and many strains producing penicillinase and cephalosporinase are still sensitive to this product. Cefixime has good antibacterial effects in vitro and in vivo against Gram-positive cocci such as pneumococci, Streptococcus pyogenes, and Gram-negative bacilli such as Haemophilus influenzae (including enzyme-producing strains), Moraxella catarrhalis (including enzyme-producing strains), Escherichia coli, Proteus mirabilis, and Neisseria gonorrhoeae (including enzyme-producing strains). Cefixime also has antibacterial activity against Streptococcus pneumoniae, Parainfluenzae, Proteus vulgaris, Klebsiella pneumoniae, Pasteurella multocida, Providencia, Salmonella, Shigella, Serratia marcescens, Citrobacter heteromorphis, and Citrobacter malonate, but its clinical effectiveness has not yet been established. This product has poor antibacterial effect on Staphylococcus, and has no antibacterial effect on Pseudomonas aeruginosa, Enterobacter, Bacteroides fragilis, Clostridium, etc.

Indication
This product is suitable for pharyngitis, tonsillitis, acute bronchitis and acute exacerbation of chronic bronchitis, otitis media, urinary tract infection, simple gonorrhea (cervicitis or urethritis), etc. caused by sensitive bacteria.
Usage and Dosage
Adults take 400 mg per day, and children take 8 mg/kg per day, either orally in a single dose or in two divided doses. Children weighing ≥50 kg or aged ≥12 years should use the adult dose. Patients with renal insufficiency whose creatinine clearance (CCr) is 21-60 ml/min and undergoing hemodialysis should be given 50% of the standard dose, i.e., 300 mg per day; patients with CCr ≤20 ml/min and undergoing peritoneal dialysis should be given 50% of the standard dose, i.e., 200 mg per day.
Adverse Reactions
Most of the adverse reactions of cefixime are short-lived and mild. The most common are gastrointestinal reactions, including diarrhea (16%), increased bowel movement (6%), abdominal pain (3%), nausea (7%), indigestion (3%), and abdominal distension (4%); adverse reactions with an incidence rate of less than 2% include rash, urticaria, drug fever, itching, headache, and dizziness. Laboratory abnormalities include transient increases in ALT, AST, ALP, LDH, bilirubin, BUN, and Cr, transient decreases in platelet and white blood cell counts and eosinophilia, and positive direct Coombs test.
Precautions
Patients with a history of allergy to this product or cephalosporin antibiotics are contraindicated.
Drug Interactions
1. This product is incompatible with the following drugs: amikacin sulfate, gentamicin, kanamycin, tobramycin. Neomycin, chlortetracycline hydrochloride, tetracycline hydrochloride, oxytetracycline hydrochloride, colistin sodium methanesulfonate, polymyxin B sulfate, erythromycin gluconate, erythromycin lactobionate, lincomycin, sulfisoxazole, aminophylline, soluble barbiturates, calcium chloride, calcium gluconate, other antihistamines of diphenhydramine hydrochloride, lidocaine, norepinephrine, metahydroxylamine, methylphenidate, succinylcholine, etc. Occasionally, it may also be incompatible with the following drugs: penicillin, methicillin, hydrocortisone succinate, phenytoin sodium. Prochlorperazine, vitamin B group and vitamin C, hydrolyzed protein. 2. The combined use of strong diuretics such as furosemide, ethacrynic acid, bumetanide, anti-tumor drugs such as carmustine, streptozocin, and aminoglycoside antibiotics with this product may increase nephrotoxicity. (4) Clavulanic acid can increase the antibacterial activity of this product against certain Gram-negative bacteria that are resistant to it due to the production of B-lactamase.
