Function and Efficacy
This product is a dibenzodiazepine antipsychotic. It has a strong blocking effect on the 5-hydroxytryptamine (5-HT2A) receptor and dopamine (DA1) receptor in the brain, and also has a blocking effect on the dopamine (DA4) receptor, and a weaker blocking effect on the dopamine (DA2) receptor. In addition, it has anticholinergic (M1), antihistamine (H1) and anti-a-adrenaline receptor effects. Extrapyramidal reactions are rare and generally do not cause an increase in blood prolactin. It can directly inhibit the ascending activation system of the brainstem reticular formation and has a strong sedative and hypnotic effect.

Indication
This product is not only effective for the positive symptoms of psychosis, but also has a certain effect on the negative symptoms. It is suitable for various subtypes of acute and chronic schizophrenia, and has a good effect on the hallucination and delusional adolescent type. It can also alleviate the emotional symptoms related to schizophrenia (such as depression, guilt, and anxiety). For some patients who are ineffective or have poor treatment effects with traditional antipsychotic drugs, switching to this product may be effective. This product is also used to treat excitement, agitation, and hallucinations and delusions in mania or other psychotic disorders. Because it causes neutropenia, it is generally not suitable as the first choice drug
Usage and Dosage
Oral administration starts with a small dose, the first dose is 25 mg (1 tablet) once, 2 to 3 times a day, and gradually increases to the commonly used therapeutic dose of 200 to 400 mg (8 to 16 tablets) a day, and the highest dose can reach 600 mg (24 tablets) a day. The maintenance dose is 100 to 200 mg (4 to 8 tablets) a day.
Adverse Reactions
1. Strong sedative and anticholinergic effects. There are many adverse reactions, including dizziness, weakness, drowsiness, sweating, salivation, nausea, vomiting, dry mouth, constipation, postural hypotension, and tachycardia. 2. Commonly seen are increased appetite and weight gain. 3. Can cause abnormal changes in the electrocardiogram. Can cause changes in the electroencephalogram or epileptic seizures. 4. Can also cause increased blood sugar. 5. Serious adverse reactions include agranulocytosis and secondary infection.
Special Population Medication
Precautions for children: Not suitable for children under 12 years old. Precautions for pregnancy and lactation: Contraindicated for pregnant women. Lactating women should stop breastfeeding while using this product. Precautions for the elderly: Use with caution or use low doses.
Drug Interactions
1. The combination of this product with ethanol or other central nervous system depressants may increase the central nervous system depressant effect. 2. The combination of this product with antihypertensive drugs may increase the risk of orthostatic hypotension. 3. The combination of this product with anticholinergic drugs may increase the anticholinergic effect. 4. The combination of this product with digoxin, heparin, phenytoin, warfarin may aggravate the bone marrow suppression effect. 5. The combination of this product with lithium carbonate may increase the risk of convulsion malignant syndrome, mental confusion and dystonia. 6. The combination of this product with antidepressants such as fluvoxamine, fluoxetine, paroxetine, sertraline may increase the plasma levels of clozapine and norclozapine. 7. The combination of this product with macrolide antibiotics may significantly increase the plasma concentration of clozapine, and there are reports of inducing epileptic seizures.
