Function and Efficacy

Penicillin has good antibacterial effect on Streptococcus such as hemolytic Streptococcus, Streptococcus pneumoniae and Staphylococcus that does not produce penicillinase. It has moderate antibacterial effect on enterococci, and Neisseria gonorrhoeae, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces bovis, Streptobacillus candida, Listeria monocytogenes, Leptospira and Treponema pallidum are sensitive to this product. This product also has certain antibacterial activity against Haemophilus influenzae and Bordetella pertussis, and other Gram-negative aerobic or facultative anaerobic bacteria are less sensitive to this product. This product has good antibacterial effect on Clostridium, Peptostreptococcus anaerobic bacteria and melanin-producing Bacteroides, but has poor antibacterial effect on Bacteroides fragilis. Penicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis.

Ingredients

Indication

Penicillin is suitable for various infections caused by sensitive bacteria, such as abscesses, bacteremia, pneumonia and endocarditis. Among them, penicillin is the first choice for the following infections: 1. Hemolytic streptococcal infections, such as pharyngitis, tonsillitis, scarlet fever, erysipelas, cellulitis and puerperal fever. 2. Pneumococcal infections such as pneumonia, otitis media, meningitis and bacteremia. 3. Non-penicillinase-producing staphylococcal infections. 4. Anthrax. 5. Clostridium infections such as tetanus and gas gangrene. 6. Syphilis (including congenital syphilis). 7. Leptospirosis

Usage and Dosage

Penicillin is administered by intramuscular injection or intravenous drip. 1 Adults: intramuscular injection, 800,000 to 2 million units per day, divided into 3 to 4 doses; intravenous drip: 2 million to 20 million units per day, divided into 2 to 4 doses. 2 Children: intramuscular injection, 25,000 units/kg of body weight, once every 12 hours; intravenous drip: 50,000 to 200,000 units/kg of body weight per day, divided into 2 to 4 doses. 3 Newborns (full-term): 50,000 units/kg of body weight each time, intramuscular injection or intravenous drip; once every 12 hours in the first week after birth, once every 8 hours for those over one week old, and once every 6 hours for severe infections. 4 Early

Adverse Reactions

1 Allergic reactions: Allergic reactions to penicillin are common, including various types of rashes such as urticaria, leukopenia, interstitial nephritis, asthma attacks, and serum sickness reactions; anaphylactic shock is rare. Once it occurs, it must be rescued on the spot, and the airway must be kept open, oxygen inhalation, and treatment measures such as epinephrine and glucocorticoids must be used. 2 Toxic reactions: Rare, but when large doses of this product are administered intravenously or intrathecally, convulsions, muscle cramps, coma, and severe mental symptoms (penicillin encephalopathy) may occur due to excessive drug concentrations in the cerebrospinal fluid. This reaction is more common in infants, the elderly, and patients with renal insufficiency. 3 Herxheimer's reaction and treatment contradictions: When penicillin is used to treat diseases such as syphilis and leptospirosis, the pathogen may die.

Precautions

It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.

Drug Interactions

1 Chloramphenicol, erythromycin, tetracyclines, and sulfonamides can interfere with the activity of this product, so they should not be dripped from the same bottle. 2 Probenecid, aspirin, indomethacin, phenylbutazone, and sulfonamides reduce the renal tubular secretion of penicillin and prolong the serum half-life of this product. Penicillin can enhance the anticoagulant effect of warfarin. 3 This product is incompatible with heavy metals, especially copper, zinc, and mercury. 4 Turbidity will appear after adding cephalothin, lincomycin, tetracycline, vancomycin, erythromycin ethylsuccinate, amphotericin B, norepinephrine, metaraminol, phenytoin sodium, hydroxyzine hydrochloride, prochlorperazine, promethazine, vitamin B group, vitamin C group, etc. to penicillin intravenous infusion. 5nbs
