Erythromycin Enteric-coated Capsules
Function and Efficacy
This product belongs to the macrolide antibiotics. It inhibits bacterial protein synthesis by binding to the 50S subunit of the ribosome of sensitive bacteria. It has antibacterial activity against Staphylococcus (except methicillin-resistant strains), streptococci of all groups and Gram-positive bacilli. Streptococcus pyogenes, group A hemolytic streptococci, Staphylococcus, Streptococcus pneumoniae, meningococci, gonorrhea, Bordetella pertussis, etc. are also sensitive to this product. This product also has antibacterial effects on various anaerobic bacteria except Bacteroides fragilis and Fusobacterium. It also has inhibitory effects on Mycoplasma pneumoniae, Chlamydia, Legionella, Treponema pallidum, Leptospira, and Campylobacter fetus. Non-clinical toxicology studies: In long-term (2-year) studies on rats, oral erythromycin has no evidence of carcinogenicity or gene mutation; studies on male and female rats have not found any effects on reproduction.

Appearance
This product is a colorless and transparent capsule, and the content is white or off-white enteric-coated erythromycin base microcapsules.
Indication
This product is mainly used for tonsillitis, pneumonia, scarlet fever, diphtheria and carriers, gonorrhea and other infections caused by G-sensitive bacteria such as Staphylococcus aureus, Streptococcus pneumoniae, and other Streptococci; it can also be used for upper respiratory tract infections (such as ear, nose, throat infections, etc.) caused by influenza bacilli, skin and soft tissue infections caused by Staphylococcus aureus, syphilis, intestinal amebiasis, etc. This product can be used as the first choice for Legionnaires' disease, mycoplasma pneumonia, etc.
Usage and Dosage
Oral administration: adults 1-2 g per day, divided into 3-4 doses.
Adverse Reactions
1 Gastrointestinal reactions include diarrhea, nausea, vomiting, gastric colic, decreased appetite, etc., and their incidence is related to the dosage. 2 Allergic reactions are manifested as drug fever, rash, eosinophilia, etc., with an incidence of 0.5% to 1%. 3 Transient elevation of AST transaminase and occasional cholestatic hepatitis occur. Common side effects include gastrointestinal reactions such as vomiting, abdominal pain, diarrhea, and poor appetite, which are related to the dosage. Allergic reactions include: urticaria, mild rash. There have been reports of temporary hearing loss in patients with poor renal function and large dosages. There have been reports that oral erythromycin can cause liver damage and jaundice.

Patients who are allergic to this product or other macrolide drugs are contraindicated
Special Population Medication
Precautions for children: 30-50 mg per kg of body weight per day for children, divided into 2 doses. Double the dose for severe infection. Or follow the doctor's advice. Precautions for pregnancy and lactation: Use with caution for pregnant and lactating women. Precautions for the elderly: No such experiment has been conducted and no reliable references are available.
Drug Interactions
1. Erythromycin can inhibit the metabolism of carbamazepine, leading to an increase in the latter's blood concentration and toxic reactions. 2. Erythromycin antagonizes the effects of chloramphenicol and lincomycin, and co-use is not recommended. 3. Patients taking warfarin for a long time may experience a prolonged prothrombin time when using erythromycin, thereby increasing the risk of bleeding. Elderly patients should pay special attention. When both must be used together, the dose of warfarin should be appropriately adjusted, and the prothrombin time should be closely observed. 4. Except for dihydroxypropylphylline, the co-use of erythromycin and xanthines can reduce the hepatic clearance of aminophylline, leading to an increase in serum aminophylline concentration and (or) increased toxic reactions. This phenomenon is more likely to occur after six days of co-use. The reduction in aminophylline elimination is proportional to the erythromycin serum peak. Therefore, the dose of xanthines should be adjusted during and after the co-use course. 5. Co-use with bromostatin will increase the latter's plasma concentration, even to toxic levels. Clinical and electrocardiographic monitoring is recommended, and the dose of digoxin should be adjusted if necessary. 6. Co-administration with cyclosporine: Erythromycin can increase the levels of cyclosporine and creatinine in the blood. When used in combination, renal function should be monitored, plasma cyclosporine levels should be measured, and the dosage of cyclosporine should be adjusted during and after treatment. 7. There have been reports of signs of ergotism with possible extremity necrosis caused by the simultaneous use of erythromycin and drugs containing ergotamine or other ergot-derived vasoconstrictors.
