Potassium Chloride Sustained Release Tablets

Function and Efficacy

Potassium is the main cation in cells, with a concentration of 150-160mmol/L, while the main cation outside cells is sodium ion, and the serum potassium concentration is only 3.5-5.0mmol/L. The body mainly relies on Na-K-ATPase on the cell membrane to maintain the difference in K and Na concentrations in cells. The acid-base balance in the body has an impact on potassium metabolism. For example, when acidosis occurs, H enters the cell. In order to maintain the potential difference inside and outside the cell, K is released outside the cell, causing or aggravating hyperkalemia. Metabolic disorders can also affect acid-base balance. The normal intracellular and extracellular potassium ion concentration and concentration difference are closely related to certain important functions of cells, including maintaining carbohydrate metabolism, glycogen storage, protein metabolism, intracellular osmotic pressure and acid-base balance, and myocardial excitability.

Appearance

This product is a sugar-coated tablet, which appears white after removing the sugar coating and contains an appropriate amount of excipients.

Indication

1 Treatment of hypokalemia Hypokalemia caused by various reasons, such as insufficient food intake, vomiting, severe diarrhea, and the use of potassium-excreting diuretics. Hypokalemic familial periodic paralysis, long-term use of glucocorticoids, and supplementation of hypertonic glucose, etc. 2 Prevention of hypokalemia When the patient has potassium loss, especially if hypokalemia is harmful to the patient (such as patients using digitalization), preventive potassium salt supplementation is required, such as eating very little, severe or chronic diarrhea, long-term use of adrenocortical hormones, potassium-losing nephropathy, and Bartter syndrome, etc. 3 Digitalis poisoning causes frequent, multi-source premature beats or rapid arrhythmias.

Usage and Dosage

Adults take 0.5-1g (1-2 tablets) at a time, twice a day, after meals, and adjust the dose according to the condition. The maximum daily dose for adults is 6g (12 tablets). Patients who have gastrointestinal reactions to the oral tablets can use the oral solution instead, diluted in cold boiled water or beverages for oral administration.

Adverse Reactions

1 Oral administration may occasionally cause gastrointestinal irritation symptoms, such as nausea, vomiting, throat discomfort, chest pain (esophageal irritation), abdominal pain, diarrhea, and even peptic ulcer and bleeding. 2 Hyperkalemia. This is prone to occur when the dosage is excessive or when there is existing renal damage. It manifests as weakness, fatigue, numbness of the hands, feet, lips and mouth, unexplained anxiety, confusion, dyspnea, slow heart rate, arrhythmia, conduction block, and even cardiac arrest. The electrocardiogram shows high and sharp T waves, and gradually prolongs the P-P interval. The P wave disappears, the QRS wave widens, and a sine wave appears. Once hyperkalemia occurs, it should be treated immediately. (1) Stop potassium supplementation immediately and avoid potassium-containing diets, drugs, and potassium-promoting diuretics. (2)

Precautions

Hyperkalemia: Patients with scant urine output and urinary retention.

Special Population Medication

Precautions for children: Not yet clear. Precautions for pregnancy and lactation: Not yet clear. Precautions for the elderly: The kidneys of the elderly have a reduced ability to clear potassium, and are more likely to develop hyperkalemia when using potassium salts. Follow-up checks of blood potassium should be performed during medication.

Drug Interactions

1. Adrenal glucocorticoids, especially those with more obvious mineralocorticoid effects, adrenal mineralocorticoids and adrenocorticotropic hormone (ACTH), can promote urinary potassium excretion, and when used in combination with this product, the efficacy of potassium salts will be reduced. 2. Anticholinergic drugs can aggravate the gastrointestinal irritation of oral potassium salts, especially potassium chloride. 3. Non-steroidal anti-inflammatory analgesics aggravate the gastrointestinal reactions of oral potassium salts. 4. When used in combination with stored blood (containing 30mmol/L potassium for less than 10 days of storage, 65mmol/L potassium for more than 10 days of storage), potassium-containing drugs and potassium-sparing diuretics, the chance of hyperkalemia increases, especially in patients with renal damage. 5.
