Usage and Dosage
Generally, 0.5% tropicamide and 0.5% phenylephrine hydrochloride mixed eye drops are commonly used in clinical practice. The combination of the two drugs has a synergistic mydriasis effect, reduces the dosage of the drug and reduces adverse reactions. 1. Mydriasis examination: This product is dripped into the conjunctival sac, 1 drop at a time, and the second drop is dripped after an interval of 5 minutes. This product begins to dilate the pupil 5 to 10 minutes after eye drops, and the pupil is dilated to the maximum in 15 to 20 minutes. After about an hour and a half, the pupil begins to shrink, and the pupil returns to the level before the drop of the drug in 5 to 10 hours. 2. Refractive examination: Use this product to drip into the eye once every 5 minutes, for 4 consecutive drops, and a refractive examination can be performed after 20 minutes. Considering the existence of residual accommodation, it is not suitable for mydriasis optometry for children under 12 years old.
Adverse Reactions
Occasionally, local eye irritation symptoms may occur. It may also cause a temporary slight increase in intraocular pressure in patients with open-angle glaucoma, but since phenylephrine itself has the effect of lowering intraocular pressure, it will not cause damage to the optic nerve.
Precautions
It is contraindicated in patients with unoperated angle-closure glaucoma.
Special Population Medication
Precautions for children: Since systemic adverse reactions are prone to occur when used by children, they should be fully observed and used with caution. In particular, there have been reports of bradycardia and respiratory arrest in premature infants. They should be fully observed and use should be stopped immediately if abnormalities are found. Appropriate treatment is recommended. The best method is to dilute the product before use as needed. Precautions for pregnancy and lactation: It should only be used when its therapeutic benefits outweigh the risks. The safety of medication during pregnancy has not yet been determined. Precautions for the elderly: Generally, the physiological functions of the elderly are reduced, and attention should be paid.
Drug Interactions
Concomitant use with monoamine oxidase inhibitors or tricyclic antidepressants can cause a significant increase in blood pressure.
