Function and Efficacy

1. This product is an angiotensin II receptor antagonist. 2. This product can selectively act on the AT1 receptor subtype known to be related to the action of angiotensin II, selectively block the binding of angiotensin II to the AT1 receptor of tissue cells such as the adrenal glands and vascular smooth muscle, inhibit vasoconstriction and aldosterone secretion, and produce a hypotensive effect. 3. The affinity of this product for AT1 receptors is about 20,000 times higher than that for AT2 receptors. This product does not affect the action of bradykinin and ion channel function, nor does it bind to the receptors of other hormones that play an important regulatory role in cardiovascular function. 4. This product has no carcinogenic, teratogenic, mutagenic toxicity, and no reproductive toxicity.

Indication

Treatment of mild to moderate essential hypertension.

Usage and Dosage

Recommended dose: Diovan 80 mg, once a day. The dose is independent of race, age, and gender and may be taken with meals or on an empty stomach (see absorption). It is recommended to take the medicine at the same time every day (such as in the morning). The exact antihypertensive effect is achieved within 2 weeks of medication, and the maximum therapeutic effect is achieved after 4 weeks. If the antihypertensive effect is not satisfactory, the daily dose can be increased to 160 mg, or a diuretic can be added. Patients with renal insufficiency (contraindications for severe renal failure) and non-biliary, non-choledostatic liver insufficiency do not need to adjust the dose. Diovan can be used in combination with other antihypertensive drugs.

Adverse Reactions

The overall incidence of adverse reactions of this drug is similar to that of the placebo group. The main adverse reactions observed in clinical applications are: headache, dizziness, viral infection, upper respiratory tract infection, cough, diarrhea, fatigue, rhinitis, back pain, nausea, pharyngitis and joint pain. The incidence of adverse reactions is not related to the dose and duration of treatment, and is not related to gender, age or race. It is not yet known whether this reaction is causally related to the treatment of this product. Laboratory indicators: This drug can occasionally cause a decrease in hemoglobin and hematocrit. In clinical controlled trials, patients taking this drug had a significant decrease in hematocrit and hemoglobin, which were 0.8% and 0.4%, respectively, and 0.1% in the placebo group. The incidence of neutropenia in patients taking this drug and taking ACE inhibitors was 1.9% and 1.8%, respectively. In clinical controlled trials, patients taking this drug had a significant increase in blood creatinine, blood potassium and total bilirubin, which were 0.8%, 4.4% and 6%, respectively, while in the ACE inhibitor group, they were 1.6%, 6.4% and 12.9%, respectively. Patients taking this drug occasionally have elevated liver function indicators. No special laboratory indicators need to be monitored when this drug is used to treat patients with essential hypertension.

Precautions

There is no experience in using Diovan in pregnant patients with severe renal failure (inosine clearance less than 10 ml/min).

Special Population Medication

Precautions for children: There is no relevant research on the effectiveness and safety of this product for children and adolescents (under 18 years old). Precautions for pregnancy and lactation: Precautions for pregnant and lactating women. Precautions for the elderly: Although the systemic exposure concentration of valsartan in the elderly is slightly higher than that in young people, it has no clinical significance.

Drug Interactions

1. Clinical trials have not found any clinically significant interactions between this drug and the following drugs: cimetidine, warfarin, furosemide, digoxin, atenolol, indomethacin, hydrochlorothiazide, amlodipine, glibenclamide. 2. This drug generally has no clinically significant interactions with metabolic inducers or inhibitors of the cytochrome P450 enzyme system. 3. In vitro tests have shown that this drug has no plasma protein binding interactions with other drugs with high plasma protein binding rates such as diclofenac, furosemide and warfarin. 4. When used in combination with potassium-sparing diuretics (such as spironolactone, triamterene, amiloride), potassium preparations or potassium-containing salt substitutes, blood potassium may increase. If they must be used together, they should be monitored carefully.
